<p>Position: RN Care Coordinator</p> <p>Department: Clinic Nursing</p> <p>Location: Glencoe, MN</p> <p>Remote Work: No</p> <p>Position Type: Part Time or Full Time</p> <p>Hours Per 2-week Pay Period: 48-64 (candidate preference)</p> <p>Shift: Days</p> <p>Schedule: Monday-Friday</p> <p>Weekend Rotation: N/A</p> <p>Holiday Rotation: N/A</p> <p>Position Summary:</p> <p>The RN Care Coordinator is a member of the Clinic Services team. This position can work in multiple areas within the clinic including Triage, Anticoagulation, Chronic Care Management, and other areas as assigned.</p> <p>Education and Experience:</p> <ul> <li>Graduate of a nursing program RN degree) required. Must be licensed in the state of Minnesota as a RN; BSN preferred. </li><li>1+ years of nursing experience required. Ambulatory nursing experience preferred. </li><li>Must have current BLS healthcare provider certification. </li><li>Strong working knowledge of Microsoft 365 applications. </li><li>Excellent verbal and written communication skills; attention to detail required. </li><li>Possesses professionalism, initiative and good judgment. Will consistently demonstrate and support Glencoe Regional Health mission and behavioral standards. </li><li>Working knowledge of patient assessment and documentation in Electronic Health Record. Excellian (EPIC) experience in a clinic setting preferred. </li><li>Must be able to meet facility required physical job requirements. </li></ul> <p>Compensation and Benefits:</p> <p>Compensation decisions are made based off of several factors including relevant work experience, education, certification and licensure as well as internal equity. It is not typical for an individual to be hired at the top of their hourly range.</p> <p>Hourly Range:</p> <p>RN (AD) $38.02-$55.27</p> <p>RN (BSN) $38.62-$55.87</p> <p>Wages are just one part of our employee compensation package.
