Our mission is to improve the health and well-being of our patients and our team members.</p> <p>PRIMARY RESPONSIBILITIES:</p> <ul> <li>Performs assessment, by obtaining patient medical and social histories, and by performing diagnostic testing and taking vital signs. </li><li>Evaluates signs and symptoms of patient and orders diagnostic tests. </li><li>Interprets and evaluates diagnostic tests and laboratory results. </li><li>Monitors and reviews patient care and treatment plans, which may include nutrition and exercise plans. </li><li>Provides education and counseling of patients and family in preventive care, medical conditions, and use of prescribed treatments and drugs. </li><li>Attends staff meetings and serves on committees as needed. </li><li>Participates in quality assurance activities. </li><li>Refers patients to physicians or other health care providers and facilities as needed and monitors outcome of the referral. </li><li>Participates in professional development activities and maintains professional affiliations. </li><li>Maintains patient confidentiality. </li><li>Oversees clinical procedures performed by nursing staff in the home setting. </li><li>Works collaboratively with Physicians in the home setting to assess high risk patients, prescribe treatment plans, and refer to higher level of care as needed. </li><li>Manage and treat chronically and acutely ill patients per protocol with IV fluids, IV and IM Medications, respiratory treatments, and other approved treatment methodologies. </li></ul> <p>REQUIREMENTS:</p> <ul> <li>Nurse Practitioner - Bachelor degree in nursing and successful completion of a nurse practitioner program at the master level </li><li>Physician Assistant - Graduate of accredited PA program </li></ul> <p>EXPERIENCE:</p> <ul> <li>Three or more years of professional experience as a nurse practitioner or physician assistant. </li><li>Prior hospital experience with preference given to providers who have worked on general medical floors or in critical care or emergency department settings. </li></ul> <p>Certificates, Licenses, Registrations</p> <ul> <li>Currently licensed in the State of Tennessee as a RN and ANCC certified as a nurse practitioner in a specialty area or licensed as a PA. </li><li>BLS and ACLS Certification required. </li></ul> <p>WHAT WE OFFER:</p> <p>State of Franklin Healthcare provides a comprehensive and competitive total compensation package designed to meet the needs of our full-time team members to include:</p> <ul> <li>Company Provided Life and Accidental Death and Dismemberment Insurance </li><li>Company Provided Long Term Disability Insurance </li><li>Employee Assistantce Program </li><li>401(k) Company Contributions </li><li>Voluntary options for Medical, Dental, Vision, and additional Life Insurance. </li><li>Company match in Health Savings Account Plans (restrictions apply) </li><li>Vacation, Personal, Sick and Holiday Time Off </li><li>An array of team member perks and discounts </li><li>Tuition Assistance Programs </li><li>And more! </li></ul>
