HCA Healthcare has committed up to 300 million in programs to support our incredible team members over the course of three years.</p> <p>Job Summary and Qualifications</p> <p>The case management assistant(CMA) will work under the direction oft the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.</p> <ul> <li> <p>CoordinateswithRNcasemanagerandsocialworkerforsupportneeded.Communicatesupdatesonprogress throughout the day. Refers patient questions back to the case manager/social worker.</p> </li><li> <p>Provides clerical support in multi disciplinary rounds, including updating NATE Tempo at direction oft team.</p> </li><li> <p>Uses web-based referral system to manage post-acute care(PAC)referrals. Includes sending appropriate clinical information from the medical record required for the level of care. The case manager or social worker will make the initial referral.</p> </li><li> <p>Monitors patients with PAC referrals and intervenes to expedite discharge.</p> </li><li> <p>PrioritizesworkloadtoensureatimelycontactandinterventionwithPACproviders.</p> </li><li> <p>Sends additional information to PAC provider when requested.</p> </li><li> <p>Communicates with RN case managers/socialworkersregardingthestatusofPACreferrals,bedavailability,and other barriers that cannot be resolved.</p> </li><li> <p>Communicates results of PAC referrals with the patient, including having the Patient Choice letter signed.</p> </li><li> <p>MonitorsMedicarepatient'splanneddischargedatesanddeliverstheImportantMedicareMessage(IMM)to the patient per facility policy.</p> </li><li> <p>Obtains physician signature on required Case Management documents and forms.</p> </li><li> <p>Based on facility practice; sets up transportation/sends information to Transfer Center and generates discharge packets for PAC transfers.</p> </li><li> <p>Utilizes Case Management systems, standards, and processes.</p> </li><li> <p>Maintains knowledge of and actively develops relationships with PAC providers.</p> </li><li> <p>Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies.</p> </li><li> <p>Practicesandadherestothe"CodeofConduct"and"MissionandValueStatement."</p> </li></ul> <p>What qualifications you will need:</p> <p>Associate Degree</p> <p>Preferred</p> <p>Medical knowledge and terminology</p> <p>Required</p> <p>Minimum of one year experience in a healthcare setting</p> <p>Preferred</p> <p>Benefits</p> <p>HCA HealthONE Aurora, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
