Explore our open positions to find a career that's the right fit for you.</p> <p>Responsibilities:</p> <ul> <li>Provide therapeutic group activities with a focus on wellness and recovery </li><li>Provide advocacy and case management services on behalf of consumers </li><li>Provide individual/family counseling </li><li>Provide liaison service from consumers to doctors </li><li>Maintain records to include progress notes, Individual Recovery Plans, discharge and transfer summaries and all other records required by Agency policies </li><li>Assist in the management of consumer finances </li><li>Active member of interdisciplinary team </li><li>Assist with transitioning consumers from state and local community hospitals </li><li>Representative in agency committee's </li><li>Provide advocacy and case management services on behalf of consumers for the on-site and off-site homes; </li><li>Schedule and/or escort (as needed) to doctor's appointments </li><li>Maintain records to include progress notes, Treatment Plans, discharge and transfer summaries, and all other records required by Agency policies and licensing standards </li><li>Be available to consumers and staff on a 24-hour basis during emergencies and flexibility to be pulled to any group home </li></ul> <p>Benefits:</p> <ul> <li>Competitive base salary </li><li>Medical and dental insurance </li><li>Vision plan </li><li>Retirement plan </li><li>Flexible spending plans </li><li>EXCELLENT time benefits for qualified positions! </li><li>Opportunity for personal and career growth, including supervision towards professional licensure for eligible candidates </li><li>Team-oriented environment - we practice the FISH! Philosophy </li></ul> <p>Qualifications:</p> <ul> <li> <p>Bachelor's (BA, BS) degree from an accredited college in a mental health related discipline OR;</p> </li><li> <p>Associate's degree plus two (2) years' experience in the human service field;</p> </li><li> <p>All positions require a valid driver's license.</p> </li></ul> <p>All positions require a valid driver's license in good standing and pre-employment drug screening.
