This position is perfect for someone who believes that healthcare should reach people wherever they are-whether that's in the clinic, in the community, or through our Mobile Health Unit.</p> <p>If you are passionate about advocacy, community impact, and building meaningful relationships with those you serve, we would love to meet you.</p> <p>What You'll Do</p> <p>In this role, you will provide compassionate case management services to individuals and families receiving care through our Outreach Programs and Mobile Health Unit, with a strong focus on supporting those experiencing homelessness.</p> <p>Your work will include:</p> <p>Supporting Community Health</p> <ul> <li> <p>Provide case management services focused on improving health and wellness outcomes for individuals experiencing homelessness</p> </li><li> <p>Develop individualized care plans in collaboration with clients and healthcare providers</p> </li><li> <p>Assist clients in overcoming barriers that impact their health, stability, and quality of life</p> </li></ul> <p>Connecting People to Essential Resources</p> <ul> <li> <p>Help clients access critical services including housing, employment, childcare, transportation, insurance coverage, Social Security, disability benefits, and other community resources</p> </li><li> <p>Assist with applications for programs such as FoodShare, BadgerCare, Medicaid, Family Planning Waiver, and Express Enrollment</p> </li></ul> <p>Advocating for Access to Care</p> <ul> <li> <p>Coordinate referrals for medical, dental, vision, behavioral health, and specialty services</p> </li><li> <p>Schedule appointments, address insurance challenges, and advocate to reduce barriers to care</p> </li><li> <p>Document services and care coordination activities within the Epic electronic medical record system</p> </li></ul> <p>Responding with Compassion in Critical Moments</p> <ul> <li> <p>Provide crisis intervention when needed, including suicide assessment, triage, and connection to mental health services</p> </li><li> <p>Facilitate referrals, records requests, and coordination with external agencies</p> </li></ul> <p>Building Community Partnerships</p> <ul> <li> <p>Support outreach initiatives including community events, health fairs, meet-and-greets, and partnership-building opportunities</p> </li><li> <p>Collaborate with the Healthcare for the Homeless team and community organizations to expand access to care</p> </li></ul> <p>Supporting Whole-Person Care</p> <ul> <li>Assist providers with brief screenings such as PHQ-9 and other required assessments </li></ul> <p>What You Bring</p> <p>Education</p> <ul> <li>Bachelor's degree in Social Work or a related field. </li></ul> <p>Experience</p> <ul> <li> <p>Previous case management or relevant community-based experience preferred</p> </li><li> <p>Knowledge of community resources and service organizations</p> </li><li> <p>Familiarity with case management practices and healthcare regulations</p> </li><li> <p>Experience using Microsoft Office and electronic medical record systems preferred</p> </li></ul> <p>Skills & Strengths</p> <ul> <li> <p>A deep commitment to serving vulnerable populations</p> </li><li> <p>Strong empathy, communication, and relationship-building skills</p> </li><li> <p>Ability to provide culturally responsive and respectful care</p> </li><li> <p>Problem-solving skills and the ability to navigate complex client needs</p> </li><li> <p>Flexibility and adaptability in dynamic community settings</p> </li></ul> <p>Language</p> <ul> <li>Spanish language skills are highly valued but not required. </li></ul> <p>Schedule</p> <ul> <li>Monday-Friday with flexibility in hours based on outreach and community needs. </li></ul> <p>Why Join N.E.W.
