<p>Purpose:</p> <p>Provides comprehensive public health nursing care to individuals and families across diverse community settings. Delivers support to pregnant individuals, infants, children, families, and communities to achieve optimal health outcomes through, nursing assessments, education, case management, and care coordination in multiple locations throughout the service area.</p> <p>Responsibilities:</p> <p>Responsibilities listed are inclusive of all Community Health programs. Nurses will be responsible for those activities specific to the assigned program.</p> <ul> <li>Provides strength-based, family-centered care in diverse settings, including but not limited to homes, WIC clinics, schools, childcare settings, and telephonic/virtual environments. </li><li>Delivers home and community-based visits to pregnant individuals, infants, children, and families, if required for the program. </li><li>Provides services as a Child Care Nurse Consultant within assigned counties, as required. Services include integrating health and safety practices into early childhood education programs by conducting safety inspections for providers and schools. Supports and coordinates care planning for children with special health care needs to ensure they can access early care and education. </li><li>Provides services such as screenings, immunizations, and testing in accordance with best practice standards and program requirements. </li><li>Utilizes evidence-based curricula and interventions, as applicable. </li><li>Promotes and maintains a safe environment for clients and infants. </li><li>Supports continuity of care among clients, families, caregivers, and healthcare providers. </li><li>Assesses assigned populations based on program, contractual, and individual client needs. </li><li>Develops, implements, and updates individualized care plans or referrals using appropriate community resources. </li><li>Provides comprehensive nursing case management and care coordination, including management of a caseload in collaboration with leadership. </li><li>Monitors client progress and communicates changes in condition or care needs to physicians and other healthcare personnel. </li><li>Educates and advises clients and families on disease prevention, reproductive life planning, maternal health (antepartum and postpartum processes and warning signs), infant care and safety, child health and growth and development. </li><li>Assists clients in establishing and maintaining medical and dental homes to ensure access to comprehensive care. </li><li>Collaborates with internal teams, community partners, and external agencies to identify needs and coordinate services. </li><li>Maintains accurate, timely, and complete clinical, billing, and program documentation in required systems and databases. </li><li>Ensures compliance with professional standards, ethical guidelines, agency policies, and program contracts. </li><li>Maintains strict confidentiality in all client interactions and records. </li><li>Represents the organization in a positive and professional manner. </li><li>Communicates effectively and respectfully with clients, team members, supervisors, and community partners. </li><li>Attends regular meetings and contributes to team collaboration. </li><li>Assists with orientation, training, and mentorship of new staff, students, and visitors. </li><li>Utilizes interpretation and translation services to provide culturally and linguistically appropriate care. </li><li>Manages time, organization, and scheduling to meet client and program needs effectively. </li><li>Engages in ongoing professional development, training, and cross-training to support a well-rounded team. </li><li>Performs other duties as assigned to support program objectives and organizational goals. </li></ul> <p>Qualifications:</p> <ul> <li>Current and valid Iowa RN license </li><li>Bachelor's degree in nursing from an accredited school of nursing preferred (required for RNs working in the Nurse Family Partnership program) </li><li>Iowa Training Project for Child Care Nurse Consultants training completion, as required after hire </li><li>Minimum of two years of clinical experience in maternal child health nursing or related field </li><li>Knowledge of professional nursing principles and practices, including knowledge of family-centered nursing care </li><li>Working knowledge of maternal child health nursing principles and practices, including community health problems, preventive measures, and community resources </li><li>Ability to plan, organize, and utilize teaching principles in working with families, clients, schools, and community organizations </li><li>Ability to exercise independent nursing judgment </li><li>Excellent critical thinking and decision-making skills </li><li>Ability to accept and work with diverse populations </li><li>Ability to communicate effectively, both verbally and in writing, using the English language </li><li>Ability to physically carry out the various aspects of providing services in the client's home, school, or other environments </li><li>Current American Heart Association Healthcare Provider certification </li><li>Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire) </li><li>Must have valid driver's license, auto liability insurance and reliable transportation if driving is required </li><li>Successful completion of pre-employment physical, drug screen, 2-step TB test and background check </li></ul> <p>Working Conditions:</p> <ul> <li>Physical requirements: Medium work exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. </li><li>Physical activities: Climbing, Kneeling, Reaching, Pushing, Talking, Hearing, Repetitive motions, Balancing, Crouching, Standing, Pulling, Stooping, Crawling, Walking, Lifting, Fingering (picking, pinching, typing, etc.), Grasping (applying pressure with fingers and palm), Feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations) </li><li>Environment: Extreme temperatures, Hazards (chemicals, infectious diseases) </li><li>Time: Ability to be flexible with a varying schedule and programmatic requirements
