<p>POSITION SUMMARY STATEMENT:</p> <p>The Breast Cancer Clinical Practice Specialist (CPS) is a nursing practice role that serves patients with breast cancer and their families throughout PNHS integrated health system. The Breast Cancer CPS identifies the needs of patients with breast cancer from diagnosis throughout the care continuum, providing consultation and collaboration with the interdisciplinary team of providers (oncology, surgery, radiation therapy, and radiology) to assist these patients and their families in achieving their health care goals. Breast Cancer CPS care includes: assessment, intervention, and ongoing evaluation of the physical and psychosocial symptoms, care needs, and outcomes of patients with breast cancer; provision of patient education and counseling; design and delivery (written and presentation) of educational materials and curricula for both patients and staff; identifying and assisting patients in obtaining health system and community resources; and utilizing as well as conducting clinical research both independently and in collaboration with internal and external researchers.</p> <p>MINIMUM QUALIFICATIONS:</p> <p>Education, Experience or Equivalent Combination:</p> <p>BSN</p> <p>Minimum of 3 years' experience in the care of oncology patients.</p> <p>Expertise in physical and psychosocial assessment.</p> <p>Experience in leadership roles, research utilization, staff development and interdisciplinary collaboration.</p> <p>Licensure/ Registration/ Certification:</p> <p>Oncology Certified Nurse (OCN) certification or ability to obtain this certification within 2 years required.</p> <p>Minnesota Nurse's License.</p> <p>Minnesota Driver's License.</p> <p>BLS provider status in accordance with American Heart Association (AHA) guidelines.</p> <p>Knowledge, Skills, and Abilities:</p> <p>Able to prioritize, delegate and be self-directed.</p> <p>Strong clinical, human relations, leadership, and verbal and written communication skills.</p> <p>Ability to effectively problem solve and make sound decisions.</p> <p>Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.</p> <p>Proficient in the operation of the following: personal computer utilizing word processing, graphics package, slide projector, overhead projector, photocopier, telephone, and fax machine.</p> <p>Proficient in pager, voicemail, and email use.</p> <p>This position requires familiarity and compliance with matters of law, regulations and internal policies affecting the employment relationship, (including but not limited FMLA, ADA, discrimination, and harassment laws).</p> <p>PREFERRED QUALIFICATIONS:</p> <p>Education, Experience or Equivalent Combination:</p> <p>Master's degree preferred.</p> <p>At HealthPartners we believe in the power of good - good deeds and good people working together.
