<p>Position Summary:</p> <p>The Health Home Quality Specialist leads continuous quality improvement efforts for the Health Home program, including performing quality reviews, conducting audits, leading quality-related projects and initiatives, and tracking and reporting program outcomes. Ensures program services provided meet organizational policies and procedures, funder requirements and accreditation standards.</p> <p>This position fulfills the Health Home Facilitator role per DMH Health Home guidelines.</p> <p>Essential Functions:</p> <ul> <li>Prepares routine program reports pulling data from a variety of systems. </li><li>Reviews program data and performs follow-up activities as needed. Ensures measures are being met at the individual service level in order to meet program goals and ensure compliance. </li><li>Completes program outcome reports that analyze program trends. Reports information in a variety of formats for in-depth analysis and program compliance measures. </li><li>Conducts program reviews to ensure documentation meets agency, funder and accreditation standards. Maintains quality-related records and documentation, and tracks activities/statuses. </li><li>Creates monthly reports on services provided by individual to ensure accurate billing. </li><li>Conducts audits and special projects as they relate to Heath Home quality objectives and the health and safety of individuals in the program. </li><li>Provides expertise, assistance, resources and training to Health Home team members as needed. </li></ul> <p>Position Competencies:</p> <ul> <li>Knowledge of disabilities, medical care and behavioral supports. Participates in organizational and funder trainings, and external training opportunities for ongoing growth and development. </li><li>Excellent computer skills. Able to access and extract data from multiple computer systems. </li><li>Able to multitask and prioritize activities daily to ensure timelines are met. </li><li>Establishes and maintains successful interpersonal relationships and works cooperatively in group situations. Able to work effectively in a team-based environment. </li><li>Intermediate-level proficiency with Microsoft Office products, including Excel, and Electronic Health Record systems. </li></ul> <p>Qualifications:</p> <ul> <li>Must have high school diploma or equivalent. Bachelor's degree in human services or related field preferred. </li><li>Two years of experience working with individuals with disabilities, or working in a healthcare setting preferred. </li><li>Must have at least one year of experience supervising or managing a caseload or project. </li><li>Must have experience using Microsoft Office suite, and intermediate-level skill with Excel. </li><li>Previous experience working with Electronic Health Record systems or other data systems preferred. </li></ul> <p>Additional Requirements:</p> <ul> <li>This position typically works weekday, daytime hours. May work evenings and weekends as need. </li><li>May travel between program offices or to community locations to attend meetings. </li><li>Must have current driver's license and proof of insurance. </li><li>Must complete and maintain all required trainings/certifications. </li><li>Must maintain confidential information in accordance with HIPAA regulations. </li></ul>
