This role offers an opportunity to contribute to meaningful work and help shape the future of care in communities across the country.</p> <p>Our Benefits:</p> <ul> <li>Comprehensive medical, dental, and vision coverage </li><li>401(k) retirement plan </li><li>Paid time off and holidays </li><li>Employee assistance programs and wellness initiatives </li><li>Flexible options to support a balanced life </li></ul> <p>Responsibilities</p> <p>What You'll Do:</p> <ul> <li>Person Centered Planning functions and Individual Support Planning Functions: </li><li>Actively participates in the person centered planning process, including development and implementation of Person Centered Plans (PCPs) and the Individual Support Plan (ISPs); </li><li>Design and implement support and teaching strategies targeted at assisting consumers with reaching their outcomes as outlined in the consumer's ISP; </li><li>Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategies; </li><li>Evaluate and revise support and teaching strategies as necessary to ensure progress toward achievement of outcomes and action steps; </li><li>Regularly monitor the implementation of the PCP and ISP; </li><li>Revise PCP and ISP outcomes as necessary based on data collection, feedback from staff, and direct observation of the consumer. Outcomes may only be changed with approval from the consumer, the consumer's guardian, and members of the interdisciplinary team. </li><li>Ensure that services and supports are provided in the PCP and ISP. </li><li>Complete monthly outcome report and provide to the Case Manager. </li><li>Facilitate opportunities for natural supports & document specific QDDP activities provided to the consumer. </li><li>Quality Assurance Related functions: </li><li>Review all forms, notify Case Manager of any incidents, and provide oversight for the follow up plan with staff. </li><li>Maintain communication with family members, guardians, staff and Case Manager on a regular basis, particularly regarding policies, issues, and concerns. </li><li>Prepare and submit reports that are required by contract, funding sources, or regulations. </li><li>Supervision/Training Related functions: </li><li>Coordinate in-service trainings needed to assist staff in fulfilling responsibilities related to consumer directed support. </li><li>Provide oversight with documentation of the Medication Administration Record with checks of the MAR at least once per week. </li><li>Monitor and record all required trainings for staff and schedule updates as needed to remain current. Complete a monthly staff schedule ensuring appropriate levels of staff coverage. </li><li>Provide direct support in providing services needed to protect the health and safety of the consumer, in a staffing crisis. </li><li>Provide training and oversight for staff regarding Medicaid Waiver guidelines and licensing and certification standards. </li><li>Communicate with the Interdisciplinary Team about concerns and findings. </li><li>Be available to respond to staff in case of an emergency. </li><li>Perform on-site evaluations for all field staff. </li><li>Complete all annual evaluations and submit to Manager one month prior to hire date anniversary to ensure timely reviews. </li><li>Other job functions: </li><li>Coordination of consumer benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps. </li><li>Securing community resources for consumers supported. </li><li>Scheduling and completion of consumer medical needs. </li><li>This description reflects assignment of essential functions, management may assign or reassign duties and responsibilities to this job at any time that are not listed above. </li></ul> <p>Qualifications</p> <p>What You'll Bring:</p> <ul> <li>Excellent organizational skills; ability to multitask and manage multiple responsibilities. </li><li>Able to supervise staff and provide necessary feedback to improve overall services. Identifies and progresses toward meeting personal and professional goals. </li><li>Strong problem-solving skills. </li><li>Strong communication skills, oral and written. </li><li>Excellent interpersonal skills. </li><li>Proficient with Microsoft Office Suite. </li><li>Access to a personal vehicle, which is insured and meets state safety/licensure requirements. </li></ul> <p>Education and Experience:</p> <ul> <li>BA/BS Degree required in social work, psychology, human services or related field. </li><li>One (1) year of experience in the field of developmental disabilities. </li><li>Supervisory experience preferred. </li></ul> <p>Management Authority:</p> <ul> <li>Conducts performance reviews </li><li>Trains other associates </li><li>Directs work of other associates </li><li>Direct workflows for entire team </li></ul> <p>Physical Requirements:</p> <ul> <li>Ability to move, transport, or position: ? up to 50 pounds; ? up to 100 pounds </li><li>Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs. </li><li>Ability to communicate effectively and clearly with others to exchange information. </li></ul> <p>Travel Requirements:</p> <ul> <li>Regular travel on a daily or weekly basis required, even in inclement weather </li></ul> <p>The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
