<p>Job Type: Regular</p> <p>Time Type: Full time</p> <p>Work Shift: Day (United States of America)</p> <p>FLSA Status: Non-Exempt</p> <p>When you join the growing BILH team, you're not just taking a job, you're making a difference in people's lives.</p> <p>No Holidays, no weekends!</p> <p>Job Description:</p> <p>Healthcare tasks performed by the CHHA include:</p> <ul> <li>Assisting patients with appropriate techniques to maintain personal hygiene and grooming, including shower, tub or bed baths and mouth, skin, nail and hair care. </li><li>Assisting patients in the use of toilet facilities, including bed pans. </li><li>Assisting patients in and out of bed using safe transfer techniques, including the use of gait belts and mechanical lifting equipment, when applicable (with appropriate completed/demonstrated skills competency). </li><li>Assisting patients with walking, including the use of walkers, canes and wheelchairs, when applicable. </li><li>Performing normal range of motion exercises and proper positioning (with appropriate completed/demonstrated skills competency). </li><li>Assisting with cuing of self-administered medications. </li><li>Assisting with meal preparation and feeding, when required. </li><li>Observing, reporting and documenting patient status and care provided. </li><li>Taking and recording of vital signs (temperature, pulse, and respirations) as directed (with appropriate completed/demonstrated skills competency). </li><li>Practice basic infection control procedures. </li><li>Document status of patient functioning and changes in patient status to be reported to the CHHA's supervisor. </li><li>Maintaining a clean, safe and healthy environment in the patient's home. </li><li>Recognizing emergencies and contacting emergency personnel and supervisor when necessary. </li><li>Understanding the physical, developmental and emotional needs of the populations served. </li><li>Maintaining adequate fluid and food intake. </li><li>Treating the patient and his/her property with respect. </li></ul> <p>Communication:</p> <ul> <li>Attends regular case conferences and team meetings per Clinical Manager and as needed. </li><li>Communicates with other disciplines on the care team and Clinical Manager regarding patient's status as needed. </li><li>Practices confidentiality principles set by the agency and federal HIPAA guidelines. </li></ul> <p>Documentation:</p> <ul> <li>All documentation will be completed in the EMR and in accordance with the agency timeliness policies. </li><li>Professional Development: </li><li>Adheres to departmental requirements for required training- Attend at least 12 hours of training annually. </li><li>Identifies specific learning needs and goals and collaborates with nursing leadership in developing a plan to meet them. </li><li>Organizational Requirements: </li><li>Home Health Aides are evaluated for competency in all areas of required CHHA training prior to delivering care to patient. The CHHA will only be allowed to provide care in areas that they have documented proof of competency. </li><li>Maintain strict adherence to the Beth Israel Lahey Health at Home Confidentiality policy. </li><li>Incorporate Beth Israel Lahey Hospital and Medical Center Standards of Behavior and Guiding Principles into daily activities </li><li>Comply with all Beth Israel Lahey Health at Home Policies. </li><li>Comply with behavioral expectations of the Home Care Division. </li><li>Maintain courteous and effective interactions with colleagues and patients. </li><li>Demonstrate an understanding of the job description, performance expectations, and competency assessment. </li><li>Demonstrate a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards. </li><li>Participate in departmental and/or interdepartmental quality improvement activities when needed. </li><li>Participate in and successfully completes Mandatory Education. </li><li>Perform all other duties as needed or directed to meet the needs of the department. </li></ul> <p>Organizational Requirements:</p> <ul> <li>Maintain strict adherence to the Continuing Care Confidentiality policy. </li><li>Incorporate Continuing Care Standards of Behavior and Guiding Principles into daily activities. </li><li>Comply with all Continuing Care Policies. </li><li>Comply with behavioral expectations of the Continuing Care Division. </li><li>Maintain courteous and effective interactions with colleagues and patients. </li><li>Demonstrate an understanding of the job description, performance expectations, and competency assessment. </li><li>Demonstrate a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards. </li><li>Participate in departmental and/or interdepartmental quality improvement activities. </li><li>Participate in and successfully completes Mandatory Education. </li><li>Perform all other duties as needed or directed to meet the needs of the department. </li></ul> <p>Minimum Qualifications:</p> <p>Education:</p> <ul> <li>Successfully completed a basic training program approved by the Mass. Association of Homemaker and Home Health Aides and possess certificate as evidence of completion. </li><li>Licensure, Certification & Registration: </li><li>Must be a licensed driver with an automobile that is insured in accordance with state. </li><li>Has an active American Health Association BLS. If no American Heart Association BLS the employee must complete an annual BILHAH BLS Competency. No other forms of BLS will be accepted. </li></ul> <p>Experience:</p> <ul> <li>One year experience as a CHHA preferred. </li><li>Skills, Knowledge & Abilities: </li><li>Meets the training requirements in accordance with State and Federal laws and possess written evidence of such completion. </li><li>At least 18 years of age. </li><li>Ability to read and follow written instructions and document care given. </li><li>Self-directing with the ability to work with little direct supervision. </li><li>Empathy for the needs of the ill, injured, frail and the impaired. </li><li>Demonstrates tact, patience and good personal hygiene. </li><li>Essential computer skills to work with EMR. </li></ul> <p>As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
