<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p> <p>Major duties include but not limited to:</p> <ul> <li>Independently completes biopsychosocial assessments, to include, diagnosing substance use, trauma related and mental health disorders, making treatment recommendations, and completing referrals. </li><li>Provides independent clinical care services including walk-in/crisis services, education, provision of self-management resources, determining and monitoring care coordination needs over time, providing care coordination, and facilitating follow-up and outreach as needed to promote proactive, personalized care and care continuity. </li><li>Independently develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team using clinical diagnostic and level of care tools to determine clinical interventions. </li><li>Provides individual, group, and family or couples therapy, utilizing evidence-based psychotherapy interventions. </li><li>Monitors progress toward treatment goals using measurement-based care. </li><li>Screens and accepts cases referred to MHC and provide mental health treatment to appropriate cases or refers cases to the best treatment site. </li><li>Provides intensive mental health services to veterans and carry responsibility for cases presenting with a wide range of psychosocial and environmental problems. Work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills. </li><li>Provides care and/or services appropriate to the age of the patients being served. Assess data reflective of the patient's status and interprets the information needed to identify each patient's requirements relative to the age-specific needs and to provide care needed as described in the services' policies and procedures. </li><li>Provides psychoeducation and information on referral services to veterans and their families. Investigate community resources and makes information about them available to medical center staff treating veterans with mental health concerns. </li><li>Provides crisis intervention and is experienced in making rapid assessments and developing crisis management plans. The incumbent has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. In the event of a medical or psychiatric emergency, the incumbent will follow the protocol of the VA Medical Center including mandatory reporting to Adult and Child Protective Services. </li><li>Works closely with VA and community health care professionals to provide and support treatment plans for veterans. Provides consultation to health care personnel on the psychosocial components of the veteran's situation. </li><li>Participates in MHC program planning, as well as in the development and maintenance of public understanding and sound working relationships with veterans, community agencies, community health care personnel and personnel from other professional disciplines. Meets with MHC staff on a regular basis. Identify gaps in services and develop VA and community resources to meet the needs. </li><li>Attends interdisciplinary team meetings, rounds, conferences for planning and coordination of services to veterans. </li><li>Participates in program/quality improvement reviews of the Mental Health Service and VASNHCS. Participates in professional peer review. </li><li>Utilizes knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. </li><li>Provides consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The incumbent provides supervision to MSW health professional trainees (HPT) and can serve as a task supervisor to other trainees. </li><li>Documents all mental health treatment and social work activity in the veteran's VA Health Care System electronic medical record. Maintain accurate, timely documentation including progress notes, encounter reports, patient data relating services provided, prepares other reports, as required, within specified deadlines. </li><li>Provides coverage to other areas of Mental Health Service or VASNHCS as needed or required. </li><li>Performs other duties as assigned. </li></ul> <p>Work Schedule: 8:00am - 4:30pm Monday - Friday</p> <p>Telework: This position is eligible for telework on an Ad- Hoc Basis</p> <p>EDRP Authorized: (Former EDRP participants ineligible to apply for incentive.)
