<p>Harbor is seeking a currently competent case manager who provides support in developing individualized goals, successful linkages with community resources, and supports participation in Benefit Bridge services that promote economic employment pathways to increased self-sufficiency.</p> <p>Position is full-time, 40 hours per week.</p> <p>Education/Experience/Other Requirements:</p> <ul> <li>Minimum of Bachelor's degree in counseling, social work or closely related behavioral/mental field from an accredited college or university, plus 1 year vocational rehabilitation experience required; or an Associates' degree plus 3 years vocational rehabilitation experience; or, in lieu of previous requirement, 5 years' experience providing vocational rehabilitation services (i.e. job development, job coaching) to BVR, ODJFS funding sources. </li><li>Preference will be given to candidate who is licensed in Ohio (LSW or LPC) and to the candidate who has experience in vocational rehabilitation with the noted participant population. </li><li>May work evenings and weekends as needed. </li><li>Must be proficient and accurate in computer use, including Microsoft Word. </li><li>CPR/First Aid and NCI/CPI Certifications required within 90 days of employment. </li><li>Must have a valid driver's license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor's vehicle insurance carrier. </li></ul> <p>Essential Job Competencies/Primary Duties:</p> <p>DOCUMENTATION</p> <ul> <li>Responsible for the quality and completeness of the electronic record. </li><li>Develops and updates an Individualized Service Plan (ISP) for each participant reflecting the seven areas of possible need: Health Care (mental health and physical health), Housing, Financial, Social Support, Leisure/Recreational, Vocational/Educational, and Crisis Prevention. </li><li>Responsible for accurate and timely documentation which includes, but is not limited to: Activity/Progress Notes, Incident Report forms and any other forms necessary to document Benefit Bridge services. </li><li>Types documentation in the format appropriate and acceptable to Harbor. </li><li>In providing billable services, fulfills or exceeds unit expectations established by Harbor. </li></ul> <p>EVALUATION</p> <ul> <li>Assesses participant "readiness" for employment and formulates appropriate interventions to assist the participant in making progress towards setting and achieving their recovery goals. </li><li>Assists the participant in identifying his/her interests, skills, and aptitudes to assist in exploring possible vocational choices. </li><li>Assists the participant in gaining information on the effects of employment on entitlements / financial assistance that the participant may be receiving (i.e., SSI/SSDI, Medicaid, Medicare, Section 8, TANF, Food Stamps) to assist the participant in making an informed choice. </li><li>Evaluates information from Financial Literacy Services and supports in developing budgets to address impact of benefit cliff. </li><li>Assesses participant bio-psychosocial needs and formulates a strength-based ISP in conjunction with the participant, family, and/or significant others. </li><li>Evaluates and revises the ISP according to the participant's responses and needs. </li><li>Assesses participant bio-psychosocial needs and formulates a strength-based ISP in conjunction with the participant, family, and/or significant others. </li></ul> <p>ONGOING INTERVENTIONS</p> <ul> <li>Participates in Benefit Bridge Care Team meetings and provides updates on participant progress. </li><li>Maintains weekly contact with all assigned participants. </li><li>Meets with participants in office and community at times and locations convenient for participant. </li><li>Assists the participant in developing employment opportunities as well as the skills to select, obtain, and maintain employment. </li><li>Promotes resiliency among participants served through identifying options available through employment. </li><li>Provides support, education (i.e., benefits counseling, employment risks and benefits, and job seeking skills) and consultation to participants, families, and/or significant others. </li><li>Actively develops employment opportunities and assists participants in learning job skills as needed, individually or in groups. </li><li>Provides therapeutic interventions as noted in the ISP to maintain the participant in the environment of choice and to enhance participant and family functioning. </li><li>Performs community outreach for the purpose of recruitment, participant engagement, and necessary support services. </li><li>Provides coaching and mentoring assistance as needed for the purpose of maintaining and retaining employment. </li><li>Carries and utilizes a cell phone for business related communication with supervisor and other staff. </li></ul> <p>COLLABORATION</p> <ul> <li>Assists the participant and family in increasing social support skills and networks. </li><li>Coordinates evaluations and assessments by internal and/or external providers. </li><li>Coordinates and monitors all services identified in the ISP. </li><li>Provides assistance in gaining access to essential community resources and linking with
