<p>Job Requirements</p> <p>POSITION SUMMARY</p> <p>The Clinic RN Hospital based is to be fully engaged in providing Quality/No Harm, Customer</p> <p>Experience, and Stewardship by providing exercise assessment, treatment planning, and cardiac</p> <p>rhythm evaluation. Under the supervision of the Medical Director, the Clinic RN oversees prescribed</p> <p>cardiac rehabilitation treatment. The Clinic RN is responsible, as a member of a multi-disciplinary team,</p> <p>for promoting optimal outcomes. The Clinic RN demonstrates an understanding of the philosophy of</p> <p>quality-driven, patient-centered care. Along with the Exercise Physiologist, the Clinic RN will</p> <p>continuously monitor patients for exercise tolerance and adverse response to exercise.</p> <p>PRIMARY ACCOUNTABILITES</p> <ol> <li>Responds immediately to any new or changing patient conditions, reporting appropriately to the </li></ol> <p>supervising and/or referring physician.</p> <ol start="2"> <li>Adheres to Health First Cardiac Rehabilitation policies and procedures in accordance with </li></ol> <p>accepted standards of practice, i.e., FL Practice Act, AACVPR Guidelines, Joint Commission,</p> <p>and Medicare.</p> <ol start="3"> <li>Documents thoroughly all initial assessment findings, treatment session results, and </li></ol> <p>reassessment outcomes, addressing clinical and behavioral data in the medical record.</p> <ol start="4"> <li>Ensures completion and certification of all components of the Individualized Treatment Plan by </li></ol> <p>the medical director or referring physician every thirty (30) days for the duration of the program.</p> <ol start="5"> <li>Contributes expertise as a member of an interdisciplinary care team, promoting cardiac </li></ol> <p>rehabilitation outcomes by demonstrating independent judgment and anticipating the needs of</p> <p>patients.</p> <ol start="6"> <li>Participates in the evaluation/initial assessment of new cardiac rehabilitation patients, </li></ol> <p>contributing to the development of the Individualized Treatment Plan with team members.</p> <ol start="7"> <li>Reviews the plan of care and goals at least every 30 days to make changes and/or reinforce </li></ol> <p>goal setting and clinical outcomes.</p> <ol start="8"> <li>Evaluates patients' risk factors, offering support and developing strategies to aid patients in </li></ol> <p>achieving successful therapeutic lifestyle changes.</p> <ol start="9"> <li>Ensures all cardiac rehabilitation patients demonstrate an understanding of the Individualized </li></ol> <p>Treatment Plan.</p> <ol start="10"> <li>Treats every patient with courtesy and respect, presenting a caring and professional tone of </li></ol> <p>voice, etiquette, and behavior when addressing goals, treatment, and overall delivery of</p> <p>services</p> <p>Work Experience</p> <p>MINIMUM QUALIFICATIONS</p> <ul> <li>Education: Associate of Science in Nursing or Nursing Diploma. </li><li>Work Experience: One (1) year in a hospital or clinic setting. </li><li>Licensure: Current RN Licensure in the State of Florida. </li><li>Certification: </li></ul> <p>o American Heart Association (AHA) Basic Life Support (BLS) Healthcare Completion</p> <p>Card prior to start date and maintained.</p> <p>o AHA Advanced Cardiac Life Support (ACLS) Healthcare Provider Completion Card</p> <p>within ninety (90) days of start date and maintained.</p> <ul> <li>Skills/Knowledge/Abilities: </li></ul> <p>o Knowledge of Cardiovascular interventions.</p> <p>o Cardiac Telemetry/ECG.</p> <p>PREFERRED QUALIFICATIONS</p> <ul> <li>Education: Bachelor of Science in Nursing. </li><li>Work Experience: Three (3) years in hospital cardiac unit or cardiology clinic setting. </li><li>Certification: American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) </li></ul> <p>Certified Cardiac Rehabilitation Professional (CCRP) within 90 days and maintained.</p> <ul> <li>Skills/Knowledge/Abilities: </li></ul> <p>o Primary Prevention Techniques.</p> <p>o Medications.</p> <p>PHYSICAL REQUIREMENTS</p> <ul> <li>Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, </li></ul> <p>or crouching frequently.</p> <ul> <li>May occasionally include lifting or moving objects up to 50 pounds, with or without assistance. </li><li>May require moving in different positions to accomplish tasks in various environments including </li></ul> <p>tight and confined spaces, including reaching out and above shoulders, or overhead.</p> <ul> <li>May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to </li></ul> <p>90% of the assigned shift.</p> <ul> <li>May be exposed to inside and outside environments with varied temperatures, air quality, </li></ul> <p>lighting, and/or low to loud noise.</p> <ul> <li>May require working with biohazards such as blood/body fluids and airborne particles following </li></ul> <p>OSHA and CDC guidelines.</p> <ul> <li>Communicating with others to exchange information. </li><li>Visual acuity and hand-eye coordination to perform tasks. </li><li>May require travel to various facilities within and beyond county perimeter; may require use of </li></ul> <p>personal vehicle.</p> <p>Benefits</p> <p>ABOUT HEALTH FIRST</p> <p>At Health First, diversity and inclusion are essential for our continued growth and evolution.
