<p>The Enhanced Care Management Case Manager is responsible for outreach, engagement, and providing support and linkage to wellness/recovery resources, both in person and virtually to assist clients in reaching the optimal level of wellness and functional capability. The ECM Case manager works with highest risk Medi-Cal members with the most complex medical and social needs, connecting with all their providers to coordinate areas of care and support.</p> <p>Duties and responsibilities</p> <ul> <li>Engages and enrolls community members eligible for ECM per care plan protocols </li><li>Assesses client need areas </li><li>Develops an individualized care plan with each client that is updated as needed </li><li>Coordinates care for clients with a multidisciplinary clinical team and outside agencies. </li><li>Facilitates referral linkages to community resources and performs case management services. </li><li>Assist clients with the development of social and community support systems. </li><li>Develops effective working relationships with agencies, organizations and providers to advocate for clients. </li><li>Documents and maintains clinical records per appropriate standards and regulations. </li><li>Conducts outreach in diverse community settings. </li><li>Participate in clinical supervision, consultations, and trainings. </li><li>Provide translation services as needed. </li><li>Complete other duties and related projects as assigned. </li></ul>
