<p>OUR VISION</p> <p>To continue as an eminent healthcare provider on Long Island, dedicating ourselves to providing exceptional health care for all our patients and to transform both the lives of the individual, and the community, for the better, one person at a time.</p> <p>OUR MISSION</p> <p>To provide access to equitable, optimal healthcare by improving the overall wellness of all individuals in our communities and delivering high quality comprehensive patient centered care.</p> <p>OUR VALUE PROPOSITION</p> <p>To provide whole person care that will ensure that all patients have access to primary, specialty and social health care to achieve and maintain optimal wellness at a transparent and affordable cost.</p> <p>JOB TITLE: HealthySteps Specialist</p> <p>REPORTS TO: Director of Behavioral Health</p> <p>The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.</p> <p>The HealthySteps Specialist, a child development and behavioral health promotion and prevention expert, joins our pediatric team and together we will follow the evidence-based guidelines of the HealthySteps model to promote nurturing parenting for the most vulnerable age group 0-3 yrs, which improves babies' and toddlers' healthy development and well-being. This aspect of the position is managed by the Director of Pediatrics.</p> <p>Additionally, this position has a clinical component where part of the schedule is devoted to conventional direct patient clinical care. In this capacity, the position is supervised by the Director of Behavioral Health.</p> <p>HEALTHYSTEPS RESPONSIBILITIES</p> <ul> <li>Promotes HealthySteps within the practice to families and providers </li><li>Conducts team-based well-child visits before, during, or following the primary care provider; visits focus on monitoring development, social-emotional functioning, and relational health,and providing anticipatory guidance </li><li>Conducts consultations on a short-term basis for needs regarding development and/or behavioral concerns such as sleep, positive parenting, picky eating, etc. </li><li>Maintains a family support line and responds to and tracks call requests within the designated response time </li><li>May facilitate parent/caregiver groups </li></ul> <p>Referrals and Community Collaborations</p> <ul> <li>Creates and maintains community resource directory/database. Provides referrals and tracks follow-up, as appropriate, to help families make successful connections to key resources within the community. </li><li>Reaches out to community providers within first 6 months in position to share about HealthySteps, their role, and receive information to support referrals, warm handoffs, and ensure appropriate referral criteria is met. </li><li>Participates (when appropriate) in community-wide early childhood and/or mental health meetings. </li></ul> <p>Team-Based Care</p> <ul> <li>Collaborates with HealthySteps implementation team to implement all eight Core Components including universal screenings, making positive parenting and early learning guidance information available, and adjusting workflows for optimal efficiency </li><li>Maintains open communication with all members of the primary care team </li><li>Works closely with pediatric primary providers around care coordination, goalsetting, coaching, and education about key aspects of a child's development </li><li>Provides consultation when appropriate with medical professionals and all practice staff re: early childhood development, early relational health/infant early childhood mental health, and trauma-informed practice. </li></ul> <p>Caseload Tracking, Record Keeping, and Reporting</p> <ul> <li>Maintains extensive databases required to meet HealthySteps fidelity metrics including both internal and external referrals. </li><li>Collaborates with HealthySteps Implementation Team to complete required annual site reporting to the HealthySteps National Office which may include analysis of both external database files and EHR data reports. </li><li>Documents all patient clinical activity and care coordination in EHR. </li><li>Tracks caseload to ensure capacity to deliver the HS services within the risk stratified service delivery model. This includes ensuring exit criteria is met and that families and children are elevated to higher levels of care or referred to community providers if needed. </li></ul> <p>PATIENT CARE RESPONSIBILITIES:</p> <ul> <li>Engages patient(s) in therapeutic process through one-on-one therapy and/or group therapy sessions </li><li>Provides therapeutic intervention to patients with behavioral health needs, including individual, family, and group psychotherapy, community work, crisis intervention, case management, other treatment modalities </li><li>Prepares psychosocial histories/reports and maintains treatment records, in accordance with clinic and regulatory guidelines </li><li>Conducts psychosocial assessments and develops and implements treatment plans </li><li>Obtains collateral information from patients, family members, outside treatment providers, schools, and other relevant team in the assessment of medical and behavioral health needs </li><li>Responds to referrals and consultation requests from primary care providers </li><li>Participates in agency Quality Assurance activities and case discussion meetings </li><li>Provides task supervision and consultation to social work interns, other clinical trainees, and volunteers if needed </li><li>Other duties as assigned. </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Master's Degree in Social Work from an accredited school of social work </li><li>New York State License required </li><li>Strong verbal and written communication skills </li><li>Previous work experience in behavioral health or health setting preferred </li><li>Ability to travel to other sites as needed </li><li>Ability to work well in teams and independently </li><li>Ability to collect and enter data for program management, evaluation, and reporting purposes </li><li>Bilingual in Spanish/and or Creole preferred </li></ul> <p>SALARY: Commensurate with experience</p> <p>MORE INFORMATION: This is an exempt position.</p>
