<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p> <p>VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU</p> <p>VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0wTotal Rewards of a Allied Health Professional</p> <p>Duties & skills needed to be successful in this position include, but are not limited to:</p> <p>Clinical Responsibilities</p> <ul> <li>Has at least two years of specialized experience that demonstrates possession of advanced practice skills and judgment. </li><li>Must be licensed or certified by a state at the advanced practice level. Provides a full range of mental health services utilizing appropriate evaluations and structured interviews, generates reports, and provides expert consultation to other disciplines or units providing care oriented to veterans at high risk for suicide. </li><li>Independently coordinate and monitor services for Veterans designated as "High Risk for Suicide", and act as the transitional coordinator of care for these patients throughout a period of the most acute risk. They manage the High-Risk Flags and ensure all local and national standards and best practices are met by all services and providers involved in the care of high risk Veterans. </li><li>Provides a full range of patient care interventions, including individual, family, and group therapy, as well as specialized interventions such as behavior management consultations and systems or environmental interventions. </li><li>Complies with all CPRS documentation procedures, including timeliness standards for documentation and proper patient check out procedures. </li></ul> <p>Complex Care Management and Consultation Responsibilities</p> <ul> <li>Follows national guidelines while independently maintaining a caseload of high-risk Veterans with complex clinical needs and providing comprehensive care management for Veterans entering a period of acute risk. </li><li>Facilitation of complex care management and risk mitigation interventions for high-risk Veterans recently discharged from the Emergency Department through the program known as SPED. </li><li>Identifying system and patient level suicide risk through the surveillance of all suicide attempts, completions and patients with serious suicide ideation across facilities in the healthcare system. </li><li>Developing and implementing an ongoing subject matter evidenced-based education and training for providers, veterans, families and members of the community on risk factors and warning signs for suicide and treatment options. </li><li>Independently evaluating Veteran progress towards goals and working with veteran and treatment providers to adjust the treatment plan, as appropriate, to best help mitigate suicide risk and in support of recovery goals. </li><li>Independently collaborates and consults in managing the complex legal and ethical issues often present in crisis intervention and suicide risk mitigation. </li></ul> <p>Consultation and Coordinator Responsibilities</p> <ul> <li>Tracking any no-show appointments by these patients and following up with the patient as soon as possible to reschedule the appointment. </li><li>Disseminating, organizing, and where possible, conducting evidence-based and promising, emerging interventions. </li><li>Assuring compliance with Joint Commission standards related to suicide prevention. </li><li>Working with community resources such as Veteran Service Organizations (VSO's) and other community groups to assure that they are aware of how to recognize veterans at risk for suicide and their options for getting these veterans assistance. </li><li>The incumbent maintains an ongoing education to provide guidance and education to providers, veterans, families and community agencies regarding suicide prevention, risk factors, assessment, intervention and treatment. </li><li>Other duties as assigned to meet the needs of the Medical Center </li></ul> <p>Work Schedule: M-F, 0730-1600</p> <p>Pay: Competitive salary and regular salary increases</p> <p>Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.</p> <p>Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #: 53864A</p> <p>Relocation/Recruitment Incentives: Not Authorized</p> <p>EDRP Authorized: Former EDRP participants ineligible to apply for incentive.
