This position serves as a central liaison between students, families, schools, and the HMHI psychiatric care team to ensure coordinated, accessible, and equitable mental health care.</p> <p>This role focuses on facilitating psychiatric consultation, supporting care coordination for ongoing mental health needs, and addressing social determinants of health that impact a child's ability to engage in treatment. The position requires both in-person work within schools and remote coordination with families and providers.</p> <p>Responsibilities</p> <p>Care Coordination & Family Engagement</p> <ul> <li>Serve as the primary point of contact for families engaged in school-based psychiatric consultation services </li><li>Obtain informed consent from parents/guardians for psychiatric evaluation and treatment services </li><li>Provide ongoing communication and support to families regarding treatment recommendations and next steps </li><li>Assist families in navigating community-based mental health resources for long-term care </li></ul> <p>School & Clinical Liaison</p> <ul> <li>Act as a liaison between school personnel and the HMHI psychiatric team </li><li>Coordinate referrals, communication, and follow-up between educators, school mental health staff, and providers </li><li>Participate in multidisciplinary meetings as appropriate </li><li>Support integration of psychiatric recommendations within the school setting </li></ul> <p>Community Resource Navigation</p> <ul> <li>Assess and address social determinants of health (e.g., transportation, insurance, housing, language barriers) that impact access to care </li><li>Connect families with community resources and supports to facilitate engagement in recommended services </li><li>Advocate for equitable access to behavioral health care across diverse populations </li><li>Support implementation of school-based psychiatric consultation services across multiple districts </li><li>Maintain accurate and timely documentation in accordance with HIPAA and institutional policies </li><li>Track referrals, service utilization, and outcomes using designated databases or registries </li><li>Assist with program coordination, workflow optimization, and quality improvement efforts </li></ul> <p>Hybrid Work Expectations</p> <ul> <li>Provide in-person support within assigned school districts when needed </li><li>Conduct remote follow-up with families, providers, and school staff as needed </li><li>Travel between school sites may be required </li></ul> <p>Minimum Qualifications</p> <p>Bachelor's degree in Health Care Administration, Business, Communication, Nursing or related field, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated human relation and effective communication skills are also required. Experience in oncology, multidisciplinary clinics, and/or working with high-acuity, complex patients may be preferred.</p> <p>This position is patient-sensitive and must fulfill all associated requirements.
