<p>Northwest Kidney Centers seeks a full-time day shift Home Program Nurse Care Manager (Home Dialysis Training Nurse) to support Federal Way East, Fife and Kent Kidney Centers in peritoneal dialysis and hemodialysis.</p> <p>Purpose</p> <p>The Home Program Nurse Care Manager (Home Dialysis Training Nurse) is accountable for the delivery and documentation of care for patients dialyzing at Northwest Kidney Centers and in their home. The Care Manager uses a collaborative practice model focusing on outcomes to enhance the quality of patient care through effective and efficient management and monitoring of patient care programs.
