Join a team where your clinical expertise, leadership, and compassion will directly shape the future of value-based care.</p> <p>What You'll Do</p> <ul> <li>As a physician leader, you will: </li><li>Deliver Purposeful Care: Practice at the top of your license across diverse settings-home, clinic, congregate living, and skilled nursing facilities-providing annual wellness visits, continuity care, urgent care, and care transitions. </li><li>Mentor and Empower: Guide a dedicated team of Advanced Practice Providers, supporting clinical decision-making, quality care delivery, and compliance. </li><li>Lead with Vision: Manage local provider teams through hiring, training, coaching, and performance evaluation, fostering a culture of excellence and accountability. </li><li>Champion New Talent: Support onboarding and clinical training for new team members, ensuring a smooth and mission-aligned integration. </li><li>Drive Value-Based Outcomes: Participate in care team huddles, contribute to individualized care plans, and promote strategies that improve outcomes and reduce unnecessary utilization. </li><li>Optimize Performance: Analyze clinical data and deploy resources strategically to meet key performance indicators (KPIs). </li><li>Collaborate for Impact: Partner with market leaders to design and implement data-driven initiatives that elevate care quality and operational efficiency. </li><li>Build Community Partnerships: Serve as a clinical liaison with hospitals, SNFs, and other providers to ensure seamless care coordination and performance improvement. </li><li>Innovate and Inspire: Lead the adoption of new clinical initiatives, technology, and workflows that enhance care delivery and team engagement. </li><li>Uphold Clinical Excellence: Adhere to evidence-based guidelines, protocols, and compliance standards to ensure safe, effective, and ethical care. </li><li>Support Around the Clock: Participate in after-hours on-call coverage as needed. </li><li>Adapt and Contribute: Take on additional responsibilities that support organizational and market-specific goals. </li></ul> <p>What You Bring</p> <ul> <li>MD or DO from an accredited institution with active board certification. </li><li>Active New York medical license, DEA registration, and state-specific controlled substance certification. </li><li>BLS certification. </li><li>Proven experience caring for aging adults and patients with complex needs. </li><li>Proficiency with clinical software and digital tools. </li><li>Reliable transportation and a clean driving record (licensed for 5+ years, no major violations or suspensions in the last 3 years, multiple violations over the past 5 years are subject to review ). </li></ul> <p>Preferred Qualifications</p> <ul> <li>3+ years of clinical practice experience. </li><li>Advanced training in geriatrics and/or palliative care. </li><li>Experience delivering care in home-based or congregate settings. </li><li>Prior leadership of clinical teams. </li><li>Background in public health, business, or health administration. </li><li>Familiarity with managed care and value-based care models. </li></ul> <p>Base Salary/ Wage Range $270,000 to $285,000 plus annual bonus. Compensation for the role is commensurate with the candidate's qualifications, skills, competencies, and experience and may fall outside of the range shown.
