<p>Position Summary</p> <p>The PT provides intermittent physical therapy in patients' homes, following physician orders and under the supervision of the Director of Nursing.</p> <p>Deliver therapy services to restore physical function, reduce pathology, promote health, and support patients in achieving productive living, as prescribed by physicians.</p> <p>Essential Duties and Responsibilities</p> <ul> <li>Conduct initial evaluations and develop treatment plans. </li><li>Administer prescribed physical therapy and monitor progress. </li><li>Write clinical notes, reevaluations, and discharge summaries. </li><li>Collaborate with physicians, families, and healthcare teams. </li><li>Supervise assistants and participate in staff training and audits. </li><li>Ensure compliance with agency policies and healthcare regulations. </li><li>Maintain accurate documentation and participate in performance improvement activities. </li></ul> <p>Knowledge, Skills and Abilities</p> <ul> <li>Strong communication, supervisory abilities, and independent judgment. </li><li>Must meet physical demands (e.g., standing, lifting). </li><li>Ability to work in stressful environments with professionalism. </li><li>Must adhere to dress code and hygiene standards. </li></ul> <p>Education & Experience</p> <ul> <li>Preferably 2 years in home/community service. </li><li>Graduate of an accredited Physical Therapy program; licensed in Texas. </li></ul>
