<p>Job Summary</p> <p>The Community Health Worker:</p> <ul> <li>Serves as a key link between patients, families, healthcare providers, and social service organizations. </li><li>Builds relationships, supports health literacy, addresses social determinants of health (SDOH), and improves access to medical and community resources. </li><li>Collaborate with internal and external partners-including medical and primary care teams-to strengthen patient care and outcomes. </li><li>Blends care coordination, health education, advocacy, and community outreach, with a focus on designated target populations, including non-English-speaking communities in identified geographic areas. Through phone outreach, clinic or hospital visits, home visits, and community engagement. </li><li>Help individuals set goals, navigate services, and maintain healthier, more stable lives. </li><li>Participate in off-site events-such as health fairs or community programs-providing outreach, health education, basic health assessments, and blood pressure screenings. </li><li>Depending on department location CHWs may need to be bilingual or be knowledgeable of specific department terminology such as medical. </li></ul> <p>Work Schedule: M-F, 8:00am-4:30pm.
