<p>Street Medicine Program Capacity Builder VISTA</p> <p>The Street Medicine Program Capacity Builder VISTA member will strengthen and expand the capacity of this community-based outreach model delivering health and social services directly to unsheltered individuals in their own environment. The VISTA member's work will focus on building systems, partnerships, and processes that ensure the program's sustainability, data-driven growth, and deeper community engagement - helping reduce barriers to care access and promote improved health outcomes for a marginalized population. The Street Medicine Program at RI Hospital is the first in RI, and partners with non-profit organizations to provide medical care to the State's unsheltered population. Over a dozen physicians from various specialties, including emergency medicine, infectious disease, and addiction medicine, provide direct medical care to patients in encampments and on street corners. Increase organizational infrastructure to support Street Medicine outreach, tracking, and volunteer engagement. 2. Build community partnerships to expand referrals, resources, and support networks. 3.
