In this impactful role, you will be instrumental in providing support to individuals within our supportive housing program.</p> <p>POSITION RESPONSIBILITIES</p> <ul> <li>Assists in securing and maintaining entitlements: SSI/SSD, PA, Medicaid, SNAP, Section 8 housing, etc. </li><li>With Community Service Coordinator (CSC), assists residents with the development and implementation of a long-range service plan </li><li>Conducts at least one monthly home visit and one phone contact with assigned residents and additional support to the resident(s) in crisis as needed and appropriate documentation of each </li><li>Responsible for contact and documentation of such with collateral service providers at least once a month and as needed </li><li>Responsible for the accurate & current maintenance of phone log and resident records </li><li>Immediately notifies CSC of all potential crisis/emergencies and documents all incidents and occurrences </li><li>Conducts resident house meetings quarterly as needed & fire drills at each residence bi-monthly </li><li>Responsible for assisting residents at the time of moving in/out and ensures that apartments are clean for new residents (may include cleaning, moving and setting up room/apartment) </li><li>Responsible for Agency compliance requirements by grants through site visits, data collection, and tracking systems </li><li>Assesses and provides referrals to residents regarding unmet service needs & reviews residents rent monthly with Supervisor </li><li>Attends and participates in staff meetings, scheduled in-service training and additional educational opportunities as well as Agency and interagency meetings </li><li>Attention to providing for the necessary development of ADL skills: budgeting, cooking, household maintenance, personal hygiene, use of public transportation, etc. </li><li>Assist in the transportation of residents to MH programs and events (court and emergency medical appointments, if necessary) </li><li>Responsible for assuring good relationships with neighbors and the general community </li><li>With CSC, responsible for discharge planning of residents </li><li>Acts as task supervisor for interns that are assigned to Supported Housing Program </li><li>Responsible for reporting maintenance issues through the appropriate process </li><li>Participates in committees, activities, events and professional groups, both internally and externally, as related to the position </li><li>Responsible for on-call duties on rotating schedule </li><li>Additional related responsibilities as needed </li></ul> <p>Enhances the direct support we bring to the families by:</p> <ul> <li>Helps facilitate children's programs/activities </li><li>Provides parent support group based on participant needs such as employment skills, education, vocational work, and life skills training </li><li>Facilitates opportunities through other community groups for education and support; </li><li>Assists CSC with quarterly documentation requirements for NYSSHP grant funding </li></ul> <p>POSITION RELATIONSHIPS</p> <p>Reports to: Community Services Coordinator</p> <p>Supervises: None</p> <p>POSITION REQUIREMENTS</p> <ul> <li>High School Diploma or Equivalency with experience in human services required </li><li>Bachelor's Degree in Human Services or related field preferred </li><li>Experience working with individuals with severe mental illness preferred </li><li>Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle </li><li>Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle </li><li>Training will take place Monday - Friday from 8:30am to 4:30pm* </li></ul> <p>SHIFT: Tuesday - Saturday (8:30 am to 4:30 pm), on-call availability</p> <p>PAY RATE: $21.00 per hour</p> <p>LOCATION: Main Office in Islip Terrace, NY</p> <ul> <li>Full Benefits Package including Medical, Company-Paid Dental and Vision Single Coverage.
