<p>The Complex Care Management Program Manager is a valuable member to our Health Care Operations who oversees the overall CCM Program. The CCM Program within the Lake County Health Centers consists of an RN Care Manager working closely with Community Health Workers to identify, engage and work with complex patients in the defined population to aid in improving their clinical outcomes, promote self-management, and decrease their need for acute care facilities such as the ED and hospitalization. The CCM Program RN Manager will meet routinely with each of the dyad CCM teams within each health center. They will review caseloads periodically and may shadow work with a patient engaged in care management. The CCM Program RN Manager is responsible for regularly reviewing patient progress and other process and outcome measures, discussing with the lead care managers, and lead care coordinators and determining programmatic or process changes needed.</p> <p>Scheduled Hours: 40 hours per week</p> <ul> <li> <p>Actively participate in the onboarding of newly hired Staff RNs, for the portion of their role that is dedicated to CCM.</p> </li><li> <p>In collaboration with the RN Managers and Staff RNs, develop and implement a training curriculum for newly hired Staff RNs, to incorporate the portion of their role that is dedicated to CCM into their other daily tasks.</p> </li><li> <p>Act as preceptor to newly hired Staff RNs, for the portion of their role that is dedicated to CCM.</p> </li><li> <p>In collaboration with the CHW Supervisor, actively participate in the onboarding of newly hired Community Health Workers.</p> </li><li> <p>In collaboration with the CHW Supervisor, develop and implement a training curriculum for newly hired Community Health Workers.</p> </li><li> <p>Maintain and ensure that all training/education documents and material are up to date.</p> </li><li> <p>Actively participate in the development, implementation, and training of new processes, policies, and protocols.</p> </li><li> <p>Monitor caseloads, process measures and outcomes over time.</p> </li><li> <p>Manage the overall CCM program at LCHD, making changes when needed, expanding eligible patients and additional dyads when needed. </p> </li><li> <p>Provide patients with resource information by utilizing local database systems.</p> </li><li> <p>Assist patients in scheduling, accessing, and utilizing health services with their primary care provider (PCP).</p> </li><li> <p>Refer patients with complex health and social needs to the appropriate community-based organization for additional support.</p> </li><li> <p>Distribute educational and health materials when appropriate.</p> </li><li> <p>Complete all required training sessions and activities.</p> </li><li> <p>Create and update personalized care plans for patients.</p> </li><li> <p>Collaborate with patients' families, friends, and social support in developing treatment plans.</p> </li><li> <p>Interact with healthcare providers to facilitate patients' treatments.</p> </li><li> <p>Ensure patients' treatment requirements are met.</p> </li><li> <p>Suggest alternative treatment plans when patients' service requests do not meet medical necessity criteria.</p> </li><li> <p>Instruct and educate patients on procedures, healthcare provider instructions, and referrals.</p> </li><li> <p>Link patients to social services programs and entitlements such as transportation assistance and translation services.</p> </li><li> <p>Conduct regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.</p> </li><li> <p>Maintain records of care management activities.</p> </li><li> <p>Develops and maintains a working knowledge of individual Emergency Preparedness role.</p> </li><li> <p>Participates in quality improvement activities to assess, monitor, and improve service area processes. </p> </li><li> <p>Reports all unusual occurrences that may involve safety and well-being of clients and co-workers directly to the supervisor and/or Safety Officer.</p> </li><li> <p>Participates in mandatory in-services and drills, attends staff meetings, and other appropriate diversity specific educational trainings offered on a regular basis for professional development.</p> </li><li> <p>All other duties as assigned.</p> </li><li> <p>Current Illinois RN license</p> </li><li> <p>Minimum five years of professional nursing experience including two years' clinical supervisory experience, preferably in a medical practice setting. </p> </li><li> <p>For internal candidates - 5 years of nursing experience. Clinical supervisory experience requirements may be offset by performance, skills, and potential demonstrated during your time as an employee.</p> </li><li> <p>Must have experience working in community work. This can include advocacy, volunteering, or community organizing. </p> </li><li> <p>Case management experience is preferred.</p> </li><li> <p>Spanish or other language fluency is preferred. </p> </li><li> <p>Flexibility to work in the evening and some weekend hours required.</p> </li><li> <p>Ability to communicate effectively with educational and culturally diverse populations in a courteous and professional manner. </p> </li><li> <p>Passion for supporting community members in protecting and promoting their health.</p> </li><li> <p>Comfort with using technology to communicate and track activities. </p> </li><li> <p>Valid driver's license and ability to travel throughout the county to various locations (vehicles available for licensed drivers).</p> </li></ul> <p>As the largest human service provider in Lake County, we believe that services must be available without barriers.
