<p>Compensation Range</p> <p>$27.08 - $35.20</p> <ul> <li>- </li></ul> <p>Job Description Summary</p> <p>The Colorado Works Case Manager provides case management by directing and coordinating services that prepare Colorado Works participants for employment or services for long-term stability. Works closely with the community to include employers, partner agencies and educational institutions to develop resources that can assist participants in obtaining and maintaining self-sufficiency.</p> <p>Primary office location is in Greeley but includes occasional travel throughout Weld County. Reliable, predictable attendance within department business hours of 8 a.m. to 5 p.m. Monday through Friday. This position is required to be in-person with a possibility for limited telework.</p> <p>Mission Statement: Engaging and partnering with the community to improve safety, health and well-being of individuals and families through the delivery of responsive and collaborative services.</p> <p>Vision Statement: The people of Weld County are connected to the resources needed to thrive in the community and feel safe and empowered.</p> <p>Weld County Department of Human Services' greatest asset is our staff.</p> <p>The Ideal Candidate for the Department of Human Services, will be someone who possesses the following traits:</p> <ol> <li> <p>Empathy and Compassion</p> </li><li> <p>Adopt a Client-Centered Approach</p> </li><li> <p>Strong Critical Thinking and Problem-Solving Abilities</p> </li><li> <p>Time management skills with the ability to stay focused with the task at hand</p> </li><li> <p>Strong communication skills in order to clearly communicate important information regularly.</p> </li></ol> <ul> <li>- </li></ul> <p>Job Description</p> <p>What you will do</p> <p>Program Support - 65%</p> <ul> <li> <p>Meet and interview Colorado Works participants to identify appropriate services and resources.</p> </li><li> <p>Coach participants on achieving short and long-term goals.</p> </li><li> <p>Develop an individualized plan that reflects the participant's interests, aptitudes, and goals alongside the participant.</p> </li><li> <p>Place participants in appropriate job readiness activities.</p> </li><li> <p>Monitor the progress and attendance in assigned activities to ensure participant is meeting program guidelines and objectives.</p> </li><li> <p>Conduct re-engagement meetings when there is non-compliance with the individualized plan.</p> </li><li> <p>Meet and evaluates participant needs on an ongoing basis and as new information is obtained.</p> </li><li> <p>Meet individual program requirements and goals such as engagement in core activities, timeliness, enrollments into work-based learning activities, and employment outcomes that contribute to meeting participation rates.</p> </li><li> <p>Regularly assess whether the current individual plan is helping the participant reach the desired immediate, intermediate, and long-term outcomes.</p> </li></ul> <p>Collaboration - 20%</p> <ul> <li> <p>Work with public and private employers and the Business Services Unit to develop work-based learning opportunities based on identified needs of the participant.</p> </li><li> <p>Collaborate with the Assessment and Learning Center staff to understand the participant's assessment results and develop learning plans to enhance their education and work-related skills.</p> </li><li> <p>Maintain a professional relationship with other units, divisions within DHS, County departments, schools, employers, service providers and community-based organizations.</p> </li><li> <p>Review requests for supportive services and authorize, or obtain, approval according to specific program guidelines and program regulations.</p> </li><li> <p>Maintain knowledge of community resources to ensure responsible and effective use of program funding.</p> </li><li> <p>When appropriate, involve community partners in service planning and meetings.</p> </li><li> <p>Share all pertinent information with partners within the bounds of confidentiality.</p> </li><li> <p>Follow up with community partners on agreed upon activities in a timely manner.</p> </li></ul> <p>Administrative - 15%</p> <ul> <li> <p>Accurately establish and maintain participant's files in accordance with Department policy and appropriate regulations, including all required documentation, employment plans, attendance and progress records and supportive service documentation.</p> </li><li> <p>Track and monitor participants activities by reviewing and entering accurate and timely information into the appropriate data management system.</p> </li></ul> <p>Required for All Jobs</p> <ul> <li> <p>Performs other duties as assigned</p> </li><li> <p>Complies with all policies and standards</p> </li><li> <p>-</p> </li></ul> <p>Required Qualifications</p> <p>Required Education</p> <ul> <li>High School Diploma/GED </li></ul> <p>Experience Qualifications</p> <ul> <li>3 years of business or clerical experience in an office environment which includes a high level of participants service and proven experience working with the public. </li></ul> <p>Preferred Education</p> <ul> <li>Associate's Degree or equivalent combination of education and experience may be considered in lieu of degree. </li></ul> <p>Preferred Experience</p> <ul> <li> <p>Experience working in a human services environment.</p> </li><li> <p>Case management experience</p> </li><li> <p>Experience working with Colorado Benefits Management System (CBMS).</p> </li></ul> <p>Skills and Abilities</p> <ul> <li> <p>Preferred ability to read and write Spanish in a proficient manner. Ability to interpret and translate between English and Spanish using professional language.</p> </li><li> <p>Candidate must demonstrate proficiency with computer technology.</p> </li><li> <p>Candidate must possess the ability to enter data into several different computer programs.</p> </li><li> <p>Candidate must possess the ability to learn and apply new computer skills on a regular basis.</p> </li><li> <p>Candidate must possess the knowledge of computer programs such as Microsoft Windows, Word, Excel, PowerPoint, OneNote, Teams, and Outlook.</p> </li><li> <p>Candidate must possess the ability to read and comprehend instructions, correspondence and department policies and procedures.</p> </li><li> <p>Candidate must possess the ability to write correspondence.</p> </li><li> <p>Candidate must possess the ability to effectively communicate in one-on-one and small group situations to participants and other employees of the organization.</p> </li><li> <p>Candidate must possess the ability to apply math concepts and calculations in the work environment.</p> </li><li> <p>Candidate must possess the ability to apply common sense to problem solve in a work environment.</p> </li><li> <p>Candidate must possess the ability to work independently and handle high-stress situations, prioritizing competing tasks or responsibilities in a fast-paced, high volume, and ever-changing work environment.</p> </li><li> <p>Candidate must possess the ability to adhere to strict confidentiality guidelines and appropriate release of information.</p> </li><li> <p>Must possess ability to keyboard at 35 words per minute.</p> </li></ul> <p>Licenses and Certifications</p> <ul> <li> <p>Candidate must pass criminal background check prior to employment start date. Required</p> </li><li> <p>Candidate must pass Department of Human Services background checks prior to employment start date. Required</p> </li><li> <p>Driving is essential in this position.</p> </li><li> <p>Candidate must have a valid Driver's License and Liability Insurance Upon Hire Required</p> </li><li> <p>Candidate must pass a Motor Vehicle Record (MVR) evaluation and if hired, will be subject to continuous monthly MVR monitoring throughout employment. Candidate must provide own transportation and be willing to travel as needed to meet the essential functions of the position.
