<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Description:</p> <p>The organ transplant coordinator assists in the care of the patient from candidate referral, evaluation, acceptance, transplant and follow-up. The coordinator is an expert in not only the care of the transplanted patient, but in advanced disease and works within a multidisciplinary team in the management of the patient in both the inpatient and outpatient setting. Included in the responsibility is outpatient clinic, telephone triage, notifying patients of lab and test results, and medication changes. Education for staff and patients, both hospitalized and in the ambulatory setting, is essential.
