Front & back of the card with expiration date.ADDITIONAL DOCUMENTS: The following documents are not required at the time of application but will be required of the selected applicant prior to employment: DOJ Clearance/FBI/CACI, Tuberculosis Skin Test clearance, Pre-Employment Physical/Drug Screen clearance. Valid California driver's license. EDUCATION AND EXPERIENCE: Any combination equivalent to: graduation from high school and two years of experience in health services, Head Start, State Preschool, or a related environment such as medical office or school nurse office. LICENSES AND OTHER REQUIREMENTS: Valid Driver's License Driving a vehicle to conduct work BENEFITS: A Benefit Entitlement for health, dental and vision coverage is available for positions that are at least 4 hours per day.
