<p>Description</p> <p>Summary:</p> <p>The LPN will provide direct and indirect care to patients working within a team model of care under the direction of an RN and in collaboration with PCTs.</p> <p>This additional role within the unit will help meet the current needs of our patients by providing additional care givers that are currently unavailable in the current market.</p> <ul> <li>9,000K Sign On Bonus! (*for eligible candidates only) </li><li>Required: Sign-On Bonus for external candidates with minimum of 1-year current LPN experience. Internal candidates who transfer to bonus eligible positions do not qualify. Bonus paid over a 14 month period of continuous full time employment. </li></ul> <p>Responsibilities:</p> <p>Provides direct patient care within the LPN scope of practice and the established model of nursing- 40%</p> <p>Documents within the EMR under the supervision of the RN. Includes vital signs, wound care, medication administration, patient observations, and patient admission to the unit.-15%</p> <p>Collaborates with the registered nurse and other members of the healthcare team to facilitate a safe and therapeutic environment for the patient-20%</p> <p>Verifies, prepares and administers medication using safe practices to assigned patients-10%</p> <p>Performs standard diagnostic procedures including EKG, phlebotomy, bladder scans and glucoscans etc. as necessary. Collects urinary specimens.-5%</p> <p>Assists in the planning, implementation and evaluation of the nursing care for patients.- 10%</p> <p>Utilizes HRO principles within the scope of daily work</p> <p>Incorporates the mission and values of Nuvance Health into daily work.</p> <p>Utilizes the Relationship Model of Care to support a positive patient experience with a focus on communication, empathy and understanding</p> <p>Participates in interdisciplinary conferences- 2%</p> <p>Participates in team building activities that contribute to patient care, staff education and self development-1%</p> <p>Maintains clinical competency to support evidence based care.5%</p> <p>Attends all mandatory and continuing educational classes-1%</p> <p>Education: HS GRAD/EQUIVALENT</p> <p>Other Information:</p> <p>Medical knowledge</p> <p>Knowledge of Infection control practices</p> <p>Critical thinking skills</p> <p>Prefer experience in a clinical setting</p> <p>Must complete an approved medication course and 40 hours of supervised medication administration including verification, preparation and administration of medication in accordance with department policy</p> <p>Knowledge and proficiency in EMR documentation systems</p> <p>Knowledge in CT DOH nursing practice requirements</p> <p>Education:</p> <p>Completion of state approved LPN program</p> <p>LPN license ( pass NCLEX-PN exam)</p> <p>BLS certified</p> <p>1-2 years recent nursing experience</p> <p>Working Conditions:</p> <p>Manual: significant manual skills/motor coord & finger dexterity</p> <p>Occupational: Some occupational risk</p> <p>Physical Effort: Very Heavy effort.
