<p>The Behavioral Health Navigator position addresses behavioral health needs through improving access to behavioral healthcare, educating and empowering patients and their families with high health needs, and improving behavioral healthcare through innovative methods of care delivery and care transition. The Behavioral Health Navigator provides professional case management services for patients by arranging assessments, connecting patients to behavioral health and community resources, developing case coordination plans, collaborating with external services to schedule services, assisting in crisis intervention, and serving as a liaison between Providence Cedars-Sinai Tarzana Medical Center and community resources.</p> <p>Providence caregivers are not simply valued - they're invaluable.
