To ensure the youth in the agency custody, over the age of 14, will be able to succeed outside of the program. This position will hold a range of responsibilities varying from employment assistance, financial management, and education assistance. The position requires strong interpersonal skills to be able to connect and create meaningful relationships with the youth(s) being serviced.</p> <p>Compensation- $42,000-$44,000</p> <p>Working at NYAP</p> <ul> <li>Flexible Schedule </li><li>22 Days of Paid Time Off + 11 Paid Holidays </li><li>Half-day Fridays throughout the summer </li><li>Healthcare Benefits for you and your family </li><li>Retirement Matching (401K) </li><li>Mileage Reimbursement </li><li>Phone Allowance </li><li>Student Loan Repayment Assistance </li><li>CEU's and ongoing trainings/education </li></ul> <p>Responsibilities</p> <p>The Transitional Living Coordinator will perform duties including, but not limited to:</p> <ul> <li>Perform a Life Skills Assessment. </li><li>Develop and Review an Independent Living Plan. </li><li>Provide Post Emancipation Services for the youth(s). </li><li>Meet with youth and supervisor(s) weekly. </li><li>Arrange and lead Family Team Meetings. </li><li>Attend monthly case conferences with supervisor(s) to discuss youth(s) progress. </li><li>Provide Independent Living Services including, but not limited to: </li><li>Academic and Post-Secondary Educational Support, including College Prep. </li><li>Employment Programs/Vocational Training. </li><li>Budget and Financial Management, including assisting in establishing banking accounts and reviewing credit reports. </li><li>Housing and Home Management Training. </li><li>Health Education and Risk Prevention. </li><li>Family Support and Health Marriage Education. </li><li>Mentoring/Supervised Independent Living. </li><li>Room and Board Financial Assistance. </li><li>Performs other duties as requested. </li></ul> <p>MINIMUM QUALIFICATIONS</p> <ul> <li>High School Diploma or GED Equivalent.
