<p>SERVICE AREA: Housing & Justice Related Services (HJRS)</p> <p>PROGRAM: Philadelphia Hepatitis Outreach Project (PHOP)</p> <p>ROLE TYPE: Part-Time</p> <p>JOB OVERVIEW:</p> <p>The Part-Time Outreach Specialist is a position within the Philadelphia Hepatitis Outreach Project (PHOP). This individual will conduct street and venue outreach throughout Philadelphia in targeted communities in partnership with other members of the HCV Street Outreach team. The focus will be on areas where there is a high level of drug trafficking and other high-risk activity to identify substance-involved women and/or men at risk for viral Hepatitis and HIV/AIDS. The main responsibility of this position is to identify and address barriers to sustained HCV treatment adherence through education, screening, behavioral health service, and linkage to treatment for individuals throughout the city of Philadelphia.</p> <p>MISSION, VISION, AND CORE VALUES:</p> <p>PHMC works to create and sustain healthier communities through partnerships with government, foundations, businesses, and community-based organizations. We envision a healthier community for all.</p> <ul> <li>Integrity: We are committed to building and sustaining trust across our teams, partners, and funders. </li><li>Impact: Our work is purposeful and rooted in a data-driven approach. </li><li>Accountability: We hold ourselves and each other responsible for getting things done, effectively managing resources, and achieving sustainable results. </li><li>Inclusion & Collaboration: We value and respect the inherent difference of all individuals, strive to amplify those voices and experiences, and are committed to working together to convene diverse perspectives, talents, and approaches to public health. </li><li>Credibility: We are a trusted resource that delivers innovative approaches and reliable services to the communities that depend on us. </li></ul> <p>RESPONSIBILITIES:</p> <ul> <li>Adhere to identified Street Outreach Safety Protocols in conducting street outreach activities. </li><li>Organize and conduct community health fairs and facilitate educational workshops and presentations throughout the community. </li><li>Interview individuals and document demographic, substance use, and health information as required by program protocols. </li><li>Link individuals to substance treatment and primary health care. </li><li>Follow up with participants who have been screened and are in need of linkage to primary or specialized health care or substance use treatment services. </li><li>Make and track referrals for identified program participants within the PHMC Client Registry System. </li><li>Travel throughout the city of Philadelphia. </li><li>Meet or exceed specified outreach productivity targets as specified by program funders and/or PHMC program managers. </li><li>Document all required program and participant demographic information into the PHMC Client Registry System within 72 hours of outreach or contact. </li><li>May be required to work some evenings and/or weekends to support program activities </li></ul> <p>SKILLS:</p> <ul> <li>Strong community outreach skills </li><li>Ability to collaborate in a team environment </li><li>Strong oral and written communication skills </li><li>Strong ability to communicate with programs within and outside of the PHMC network </li><li>Demonstrated ability to work with substance-involved individuals </li><li>Knowledge of substance use and abuse </li><li>Knowledge of viral Hepatitis and HIV/AIDS </li><li>Knowledge of public health, social services, resources, and agencies in the community </li><li>Knowledge of Hepatitis education, screening, and group facilitation </li><li>Familiarity with the project's target area and population </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>High school diploma or GED required </li><li>Preferred: Associate degree </li><li>At least two years of experience in public health or social service-related work </li><li>At least two years of experience working with diverse populations, low-income individuals, and individuals from different religious backgrounds </li><li>Preferred: Bilingual skills </li></ul> <p>PAY GRADE: 14</p> <p>PHMC is an Equal Opportunity and E-Verify Employer.</p>
