<p>Position Summary:</p> <p>The Quality & Performance Improvement (QPI) Specialist is a valuable member of the Nursing Department who assumes a variety of duties. Under the direction of the QAPI Coordinator, or designee, the QPI Specialist will perform day to day operations and technical functions of the Quality Assurance Department. Qualified candidates will demonstrate superior communication and organization skills while adhering to our high standard of excellence through the LISVH mission, values and code of conduct</p> <p>Duties of a Quality & Performance Improvement Specialist may include the following but are not limited to:</p> <ul> <li>Organizes all QA documents and prepares for administrative reviews. </li><li>Tracks and trends all QA events and presents results during monthly and quarterly QAPI meetings. </li><li>Reviews all completed QA event documents to ensure completion, return incomplete documents to assigned staff member with detailed notes and explanation to ensure that staff have a full understanding of deficient completion. </li><li>Refers staff members with repetitive deficient completion to ADON and/or DON for a corrective action plan. </li><li>Distributes and monitors for completion of assigned monthly / quarterly audits. Reviews audits prior to submission to QAPI coordinator to ensure completion. </li><li>Completes facility Accu-Shield audit to ensure compliance with COVID-19 protocol. Reports results to the QAPI committee during monthly and quarterly meetings. </li><li>Participates in the review and revision of facility policies and procedures to ensure adherence to QAPI standards and facility QAPI plan. </li><li>Identifies concerns related to the Medical Record, which could benefit from the implementation of a performance improvement project (PIP). Participates in facility PIPs, as directed. </li><li>Participates in staff orientation and in-service programs as required. </li><li>Performs departmental administrative duties under the direction of the Director of Nursing. </li></ul> <p>Required Qualifications:</p> <p>Bachelors Degree in a Health Science Major with two (2) years of experience working with the geriatric population in an administrative capacity. In lieu of Bachelor's degree, an Associate's degree in a relevant field with four (4) years of experience working with the geriatric population in an administrative capacity. Minimum of one-year (1) experience working with quality assurance and/or performance improvement projects. Three (3) years of experience working with an Electronic Medical Records System. Working knowledge of HIPAA, privacy and release of information regulations. Excellent verbal and written communication skills. Excellent proficiency in Microsoft Office Suite programs, as well as data analytical tools such as but not limited to; REDCap, SPSS, Qualtrics, and Pointright.</p> <p>Preferred Qualifications:</p> <p>Three (3) years of experience working within a QAPI role. Three (3) years of experience working in a Skilled Nursing Facility. Experience working with the Department of Veterans Affairs (VA) system. Demonstrated excellence in verbal and written communication, as evidenced by a scholarly paper or article written within the past two (2) years. QAPI Certification from an accredited organization.</p> <p>Resume/CV and cover letter should be included with the online application.</p> <p>Posting Overview: This position will remain posted until filled or for a maximum of 90 days. Please note, that if no candidate was identified and hired within 90 days from initial posting, the posting would close for review, and possibly reposted at a later date.</p> <p>Working Conditions:</p> <p>Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue as well as other unpleasant elements such as accidents, injuries and illness. May be exposed to and be required to handle varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries.</p> <p>Special Notes:</p> <p>The Long Island State Veterans Home is a tobacco-free environment. Tobacco products are strictly prohibited anywhere on our 25-acre campus, including parking lots and outdoor areas on the premises.</p> <p>Essential Position: This position has been designated as an essential position based on the duties of the job and the functions performed. This means that when the Long Island State Veterans Home is faced with an emergency, employees in such positions are required to remain at their work location or to report to work.</p> <p>The LISVH is a 24/7 skilled nursing facility. As such, all positions are subject to changes in pass days and shifts as necessary to meet the operating needs of the facility.</p> <p>The use of Vacation or Personal Leave accruals during the first 6 months of employment will not be authorized unless prior approval has been granted by Human Resources.</p> <p>The selected candidate must successfully clear a background investigation. Prior to start date, the selected candidate must meet the following requirements: Successfully complete pre-employment physical examination and obtain medical clearance from the Long Island State Veterans Home Employee Health Department, clear a background investigation, submit three (3) written references, and provide a copy of the required license(s)/certificate(s). Please be advised that failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.</p> <p>In accordance with the New York State Department of Health (DOH) order that all hospitals and nursing homes "continuously require all covered personnel to be fully vaccinated against COVID-19." Upon acceptance of a conditional job offer, candidates who are not already vaccinated must obtain the first dose of the vaccine within three (3) calendar days of job offer and must obtain any subsequent doses in accordance with the vaccine protocol.
