Flexible scheduling options are available.</p> <p>The Oncology Nurse Navigators helps patients navigate their care and treatment, from their initial referral through to survivorship. They work closely with our team of providers and other interdisciplinary teams across Mount Auburn Hospital. The role also involves triage, basic office nursing such as port draws, and handling medication authorization and funding for oral chemotherapy. This role is ideal for anyone who wants to work with a close-knit team and see patients through the entire course of the treatment journey!</p> <p>Job Description:</p> <p>Description:</p> <p>The Medical Oncology Nurse Navigator is a professional registered nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Utilizing the nursing process, the Medical Oncology Nurse Navigator provides education and resources to facilitate informed decision making and</p> <p>timely access to quality health and psychosocial care throughout all phases of the cancer continuum.</p> <p>Working within the Hematology/Oncology Outpatient Practice setting the Oncology Nurse Navigator provides patient-centered care to patients of all cancer types and benign blood disorders. The Oncology Nurse Navigator works as part of a dynamic multidisciplinary team of professionals that includes, but is not limited to; MDs, NPs, RNs, Medical Assistants, Pharmacists, and Social Workers.</p> <p>The Oncology Nurse Navigator functions as an integral member to the team helping ensure patients receive quality and compassionate care. Skills such as clinical decision making, critical thinking, effective communication, and nursing assessment and symptom management specific to the oncology nurse are necessary for this role.</p> <p>Responsibilities</p> <p>Is responsible for coordinating care for patients for a defined population (for example patients receiving oral chemotherapy), including the evaluation, assessment, scheduling, consultation and treatment of patients in order to implement a seamless progression of care to meet the patient's needs</p> <p>Provides a welcoming, helpful approach to new MAH patients, promotes positive, professional image</p> <p>Provide patient/family education regarding chemotherapy/immunotherapy</p> <p>Coordinate care for patients preparing to start treatment including pre-tx staging, labs, other testing and venous access placement</p> <p>Collaborate with colleagues within MAH in the care of the medical oncology patients</p> <p>Support Hematology/Oncology physicians and nurse practitioners with telephone triage, medication Rx renewals and outside lab ordering. In addition, collaborates with physicians and other health care providers to coordinate the care for patients from referral/consultation to discharge /follow up</p> <p>Place referrals to and collaborate with our community agencies such as VNA, Hospice and other home care agencies Collaborate with and make referrals to specialists including Cardiology, Endocrine, Genetics, Nutrition, Social Work, Physical Therapy and Palliative Care Serves as contact and facilitator to receive consults and patient information Coordinates and insures all patient scheduling including but not limited to consultation, diagnostic and treatment procedures Gathers information for history and physical Performs and documents evaluations and assessments of patient Assists in development of patient/family educational materials Participates in patient rounds and case conferences Updates clinical knowledge and skills through consultation, formal and informal education, and review of literature for self and clinical team Provides leadership for process improvement related to the care of these patients Acts as a liaison between the patients, families, and caregivers and the providers to optimize outcomes Makes and answers patient phone calls, both pre and post treatment/procedure, from patients referring providers, and outside facilities (hospitals, rehabilitation centers, physician offices)</p> <p>Qualifications ·</p> <ul> <li>Licensed Nurse in the state of Massachusetts · </li><li>BSN Required · </li><li>BLS certification required. · </li><li>Two years of nursing experience in an oncology setting, preferred but willing to train · </li><li>ONS Chemotherapy / Biotherapy Certified preferred, or obtained within 2 years of hire · </li><li>ONC certification preferred or obtained within 2 years of hire. </li></ul> <p>Decision Making: Ability to make decisions that are guided by precedents, policies and objectives. Regularly makes decisions and recommendations on issues affecting a department or functional area</p> <p>Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.</p> <p>Written Communications: Ability to summarize and communicate in English moderately complex information in varied written formats to internal and external customers</p> <p>Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations Team Work: Ability to act as a team leader for small projects or work groups, creating a collaborative and respectful team environment and improving workflows</p> <p>Independence of Action: Ability to set goals and determines how to accomplish defined results with some guidelines</p> <p>Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s)</p> <p>Resolves service issues in the assigned unit(s) in a timely and respectful manner</p> <p>As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
