<p>SUMMARY:</p> <p>Collaborates with primary care providers and interdisciplinary care team to provide accessible, innovative, and patient-centered care to high-risk patients with diabetes. Primarily through telephonic follow-up, empowers patients to manage their diabetes by providing education, leveraging technology, and tailoring support to individual needs.</p> <p>In addition to our values, all employees are expected to demonstrate the core Success Factors which tell us how we work together and how we get things done. The core Success Factors include:</p> <p>Instill Trust and Value Differences</p> <p>Patient and Community Focus and Collaborate</p> <p>RESPONSIBILITIES:</p> <ul> <li> <p>Conducts patient outreach to assess needs, ensure completion of recommended diabetes-related screenings, establish a self-management care plan, and provide continued support.</p> </li><li> <p>Utilizes digital tools and emerging technology to facilitate patient self-monitoring and support remote glucose monitoring for the care team.</p> </li><li> <p>Coordinates and conducts in-person or telehealth visits for continuous glucose monitoring (CGM) education, data downloads, and other patient needs.</p> </li><li> <p>Evaluates the healthcare, educational, and psychosocial needs of patients, families, and caregivers, connecting them with appropriate resources.</p> </li><li> <p>Provides diabetes education to patients newly diagnosed with diabetes.</p> </li><li> <p>Assists with care coordination related to diabetes management during and after care transitions.</p> </li><li> <p>Monitors patients' diabetes management plans and collaborates with primary care physicians to optimize treatment.</p> </li><li> <p>Contributes to ongoing program development while promoting and adhering to ADA Standards of Medical Care in Diabetes.</p> </li><li> <p>Builds and maintains collaborative relationships with external healthcare providers and facilities.</p> </li><li> <p>Strengthens coordination and communication with other clinical programs and healthcare partners.</p> </li><li> <p>Ensures compliance with federal and state confidentiality laws, including HIPAA, to safeguard patient privacy.</p> </li><li> <p>Adheres to Brown Health Medical Group Primary Care policies and guidelines related to protecting patient demographic, clinical, and financial information.</p> </li><li> <p>Performs additional duties as assigned.</p> </li></ul> <p>MINIMUM QUALIFICATIONS:</p> <ul> <li>Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing. </li></ul> <p>Education and Experience</p> <ul> <li> <p>Graduate of an accredited school of nursing with a current Rhode Island RN license in good standing</p> </li><li> <p>Minimum three years of experience in direct patient care, including experience in diabetes management</p> </li><li> <p>CDCES or CDOE certification or willingness to obtain within 1 year of hire</p> </li><li> <p>An equivalent combination of education and experience may be considered.</p> </li></ul> <p>Knowledge, Skills and Ability</p> <ul> <li> <p>Strong interpersonal skills and the ability to collaborate effectively to achieve shared goals.</p> </li><li> <p>Excellent organizational and prioritization abilities, with strong multitasking and project-management skills.</p> </li><li> <p>Working knowledge of medical terminology and understanding of medical insurance and benefits.</p> </li><li> <p>Advanced proficiency with electronic health records (EPIC), Microsoft Office applications (Word, Excel, Outlook), and web-based platforms.</p> </li><li> <p>Demonstrated critical thinking, problem-solving abilities, and sound clinical judgment.</p> </li><li> <p>Ability to thrive in a fast-paced environment and adapt to changing needs.</p> </li><li> <p>Ability to actively participate in quality assurance and continuous improvement initiatives.</p> </li><li> <p>Ability to work both independently and as part of a team, with strong self-direction.</p> </li><li> <p>Strong work ethic and commitment to supporting the well-being of patients and families.</p> </li><li> <p>Ability to communicate with compassion while maintaining professionalism and composure.</p> </li><li> <p>Competence in performing standard clinical procedures in accordance with established protocols.</p> </li></ul> <p>Physical Effort and Dexterity</p> <ul> <li> <p>This role works primarily in a remote capacity and has daily communications through virtual meetings, some time in the office may be required for meetings, trainings, patient visits etc.</p> </li><li> <p>Sit and stand for prolonged periods of time</p> </li><li> <p>Excellent hand/eye coordination</p> </li><li> <p>Lift up to 15 pounds</p> </li><li> <p>Manual dexterity to operate a personal computer or laptop as well as standard office machines and equipment as it pertains to one's position</p> </li><li> <p>State-wide travel and operation of motor vehicle may be required for offsite meetings and trainings</p> </li><li> <p>Must have a valid driver's license and clear driving record</p> </li><li> <p>Use of personal vehicle and proof of auto insurance is necessary</p> </li></ul> <p>Visual Acuity, Hearing and Speaking</p> <ul> <li>Capability to transfer information from original source to paper, computer, telephone and in
