By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.</p> <p>As a Home Health Physical Therapist, you will:</p> <ul> <li>Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function. </li><li>Assess and document patients' physical strengths to support physician evaluation and guide treatment goals. </li><li>Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team. </li><li>Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. </li><li>Document patient observations, interventions, and evaluations promptly and thoroughly. </li><li>Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries. </li></ul> <p>Use your skills to make an impact</p> <p>Required Qualifications:</p> <ul> <li> <p>Degree from an accredited Physical Therapy Program (approved by the APTA)</p> </li><li> <p>Current and unrestricted Physical Therapy license</p> </li><li> <p>Current CPR certification</p> </li><li> <p>Experience collaborating with a team of healthcare professionals</p> </li></ul> <p>Preferred Qualifications:</p> <ul> <li>One year of experience as a Physical Therapist </li></ul> <p>Pay Range</p> <ul> <li>$59.00 - $83.00 pay per visit/unit </li><li>$93,000 - $128,000 per year base pay </li></ul> <p>Additional Information</p> <p>TB Statement:</p> <p>This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.</p> <p>Driving Statement:</p> <p>This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.
