<p>Full Time (72 Hours Biweekly)</p> <p>Shift: Evening (11:00 a.m. - 11:30 p.m.)</p> <p>Every other weekend and every other holiday</p> <p>Ephrata Emergency Department</p> <p>Ephrata Community Hospital's emergency department will see on average 30,000 patients a year. The emergency department is a 29-bed department including a 6-bed secure behavioral health area.</p> <p>General Summary</p> <p>Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the Hospital. Compiled history information is entered into the patient's electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care.</p> <p>Duties and Responsibilities</p> <p>Essential Functions:</p> <ul> <li>Collects an accurate best possible medication list of the patient's home medications in anticipation of hospital admission. </li><li>Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospital's electronic health record. </li><li>Effectively communicates with other health care professionals to prioritize patient care and identify patient specific needs. </li><li>Accurately enters the home medication list into the Hospital's information system for utilization by the provider responsible for admitting the patient. </li><li>Updates the medication list in a timely manner so it can be used as the foundation for the patient's admission medication orders. </li><li>Directs any unresolved issues to the admitting provider. </li><li>Gathers medication information for patients in the ED and those being directly admitted to the floor. </li><li>Greets, directs and assists patients, visitors, physicians and ancillary department staff. </li><li>Answers telephones using personable etiquette. Answers inquiries by providing accurate information or forwarding callers to the appropriate resource. Identifies emergency messages, directs information to the appropriate personnel and maintains confidentiality. </li><li>Communicates delays/difficulties in reconciliation process with appropriate disciplines. </li><li>Assists with care and maintenance of department equipment, hardware and supplies. </li></ul> <p>Common Expectations:</p> <ul> <li>Demonstrates a commitment to patients, visitors, and staff by: complying with all applicable safety regulations; learning the impact of duplication errors and methodology that will lead to a reduction of errors; reporting actual and potential errors as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety and participating in safety and compliance education and root cause analyses as required. </li><li>Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. </li><li>Demonstrates interpersonal communication to enhance/maintain good relationships. Takes a proactive approach to solving problems or behaviors. Displays a positive attitude as demonstrated through verbal and non-verbal communication. </li><li>Demonstrates behavior consistent with System mission, vision, goals, objectives and patient care philosophy. </li><li>Maintains effective communication and good relationships with other discipline within the facility for the patient's benefit. </li><li>Treats all individuals with respect, courtesy and empathy. Willingly assists others with questions. Introduces self to visitors and offers assistance. Verbal and non-verbal responses are courteous, friendly and caring. </li><li>Participates in educational programs and in-service meetings. </li><li>Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. </li></ul> <p>Qualifications</p> <p>Minimum Education:</p> <ul> <li>High School Diploma or GED Required </li></ul> <p>Work Experience:</p> <ul> <li>Less than 1 year 3 - 6 months Required </li><li>Experience with medication names, their uses, doses and routes of administration. Preferred </li></ul> <p>Courses and Training:</p> <ul> <li>4 weeks hands-on training in the department. Upon Hire Required </li><li>Medical Terminology.
