<p>Shift: Days, M-F 8AM-4:30PM</p> <p>GENERAL SUMMARY:</p> <p>Responsible for assessment, plan, intervention and evaluation of Basic and Comprehensive Diabetes Self -Management Training to all patients referred to Florida Hospital Memorial Medical (FHMMC) Diabetes Center. Maintains current knowledge of diabetes and adheres to all processes according to the American Diabetes Association (ADA) National Diabetes Education Standards and FHMMC ADA Program Recognition standards. Provides optimum diabetes education and group instruction to patients from adolescents through geriatric age. Requires travel between multiple hospitals and scheduled community outreach events.</p> <p>PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:</p> <ul> <li>Demonstrates comprehensive understanding of diabetes mellitus and gestational diabetes.• Regularly works with medical staff in multiple hospitals to provide comprehensive diabetes education training and skills for referred outpatients. • Conducts initial assessment and reassessment of patient/family's education needs and develops a specific plan of care tomeet those needs.• Revises plan of care as indicated by the patient's response; evaluates overall teaching plan for effectiveness in achieving optimal established goals and optimal outcome.• Utilizes and coordinates resources to provide appropriate continuum of care for the patient/family.• Provides dynamic one-on-one instruction and group instruction both in outpatient and inpatient setting.• Coordinates, conducts and facilitates diabetes support group meetings• Provides in-services and continuing education programs to meet identified/justified needs.• Adequately maintains supplies for education program needs.• Uses audio-visual equipment and aides• Obtains information and continuing education necessary to provide current diabetes education• Demonstrates appropriate and age-related assessment and intervention. </li></ul> <p>Develops, reviews and revises the Diabetes Program Curriculum on an annual basis or as needed to provide current andaccurate information.• Completes required documentation as needed for timely and accurate patient registration and service charges• Complete documentation in the patient's medical record and Diabetes DataTrak according to department standards and ADA Program Education Recognition Standards.• Provides timely and accurate responses to telephone calls from patients/family and other health professionals.• Understands and utilizes appropriate patient referral, scheduling and appointment processes in the department.• Able to perform computer skills required for Diabetes Education Program, Diabetes DataTrak, and email communication within department.• Member and active participant in the FHMMC Diabetes Program Advisory Committee.• Recommends and develops protocols and clinical pathways to ensure quality diabetes care.• Demonstrates time management and organization skills for completing daily, weekly and long-term assignments, including traveling and set-up and clean-up for assignments.• Participates in community outreach events as defined by department and markets diabetes management services to the medical community and community at large.• Provides professional expertise to other members of the diabetes care team, nursing staff and community.</p> <p>Qualifications</p> <p>EDUCATION AND EXPERIENCE REQUIRED:</p> <ul> <li> <p>Associates in Nursing</p> </li><li> <p>Minimum of 4 years clinical experience.</p> </li><li> <p>CDE required</p> </li></ul> <p>PREFERRED:</p> <ul> <li>BS degree preferred in Nursing or Education specialty </li></ul> <p>LICENSURE, CERTIFICATION OR REGISTRATION:</p> <p>REQUIRED:</p> <ul> <li> <p>Current Florida RN license</p> </li><li> <p>Driver's License required</p> </li></ul> <p>This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
