<p>Job Title: Licensed Practical Nurse (LPN)</p> <p>Job Description</p> <p>The Licensed Practical Nurse (LPN) is responsible for providing routine nursing care, accurate documentation, and thorough record reviews. This role involves overseeing medical information, ensuring follow-up actions are documented, performing chart reviews, and scheduling appointments.</p> <p>Responsibilities</p> <ul> <li>Conduct comprehensive chart reviews. </li><li>Ensure all documentation is accurate and follow-up actions are noted. </li><li>Assist in maintaining files related to the medical care of individuals as outlined in the Table of Contents. </li><li>Carry out charting functions for all medical matters related to individuals, including recording medical concerns and other pertinent information. </li><li>Complete and ensure the completion of routine tasks such as processing doctors' orders, scheduling follow-up appointments and tests, maintaining ticklers, face sheets, BM charts, self-medication checklists, and scheduling necessary appointments. </li></ul> <p>Essential Skills</p> <ul> <li>Licensed Practical Nurse (LPN) credential. </li><li>Proficiency with electronic health records. </li><li>Experience in healthcare, patient care, medical documentation, and patient education. </li></ul> <p>Additional Skills & Qualifications</p> <ul> <li>LPN License. </li><li>3 years of LPN experience. </li><li>Interest in working with underserved communities. </li></ul> <p>Work Environment</p> <p>This position offers a Monday through Friday schedule from 8 AM to 5 PM, totaling a 35-hour work week.
