<p>Overview</p> <p>The Home Infusion RN is a member of the YNHH Home Infusion Therapy team. Responsible for the delivery of</p> <p>high-quality infusion therapy and nursing services in the home or alternate settings (i.e.; Ambulatory Infusion</p> <p>Suites).This includes performing patient assessments, managing various vascular access devices, and educating</p> <p>patients and/or caregivers to ensure safe and effective treatment according to physician 's plan of care. Provides</p> <p>education to patient/and/or caregiver on proper handling, storage and administration of infusion medications as well</p> <p>as signs and symptoms of access device complications.</p> <p>EEO/AA/Disability/Veteran.</p> <p>Responsibilities</p> <p>Provides nursing services at home and/or alternate sites for intravenous, subcutaneous, oral, etc. medication</p> <p>administration for YNHHS Home Infusion Pharmacy patients in compliance with YNNHS policies.</p> <p>Works in collaboration, coordinates patient care, and maintains communication with the physician, pharmacy</p> <p>team, and other members of the collaborative treatment team.</p> <p>Teaches patients and/or family/caregivers about the patient's care needs (including self-administration when</p> <p>applicable) and disease prevention</p> <p>Maintains accurate and timely clinical nursing records according to YNHHS Home Infusion Pharmacy</p> <p>policies.</p> <p>Adheres to ethical nursing standards and maintains the confidentiality of patients and their personal</p> <p>information at all times.</p> <p>Perform other duties as directed by home infusion leadership to support the needs of the department and</p> <p>represents the home infusion pharmacy in a professional and positive manner.</p> <p>Stay abreast of developments in the field of infusion therapy and comply with all YNHHS policies and</p> <p>directives for the fulfillment of the YNHHS objectives</p> <p>Participates in staff meetings, in-service programs, and serves on committees as requested by the Manager</p> <p>of Home Infusion.</p> <p>Follows and adheres to infection control measures and standards for home care nursing and IV nursing</p> <p>services</p> <p>Plays a significant role in the development, evaluation and implementation of standards of practice and related</p> <p>policies and procedures, ensures compliance with regulatory and accreditation standards relating to nursing.</p> <p>Qualifications</p> <p>EDUCATION:</p> <p>Nursing diploma or associate degree in nursing. Bachelor's degree in nursing preferred. CRNI certification a plus.</p> <p>EXPERIENCE:</p> <p>Three years of IV related experience. Previous experience in the home care or home infusion setting is desirable . Case Management experience a plus.</p> <p>SPECIAL SKILLS: Experienced in the care of peripheral and central lines. The ability to troubleshoot potential problems that may arise</p> <p>in dealing with IV therapy. Knowledge of medications, indications, dosage ranges, side effects, and potential</p> <p>toxicities. Ability to work independently. Must have excellent customer service skills (both verbal and written) and</p> <p>have effective interpersonal skills.
