<p>The Post-Acute Care Admission Coordinator Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. They are an active member of an interdisciplinary team whose primary focus is on the Community Living Center's (CLC) screening and placement process, and have a holistic understanding of the admission criteria of each CLC program area.</p> <ul> <li>Responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. </li><li>Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. </li><li>Collaborate with the members of the healthcare system and stakeholders to provide a seamless, timely referral system to Veterans seeking admission to the CLC. </li><li>Collaborate with both referring and accepting staff ensuring that Veterans are placed in the program area that can best meet their needs. </li><li>Assist with patient planning assessments, identification of barriers, and activities required to successfully transition the patient into the post-acute setting. </li><li>Monitor bed availability and serves as the point of contact for the outcome of screening decisions. </li><li>Coordinate and collaborate with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed. </li></ul> <p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards</p> <p>Pay: Competitive salary, regular salary increases, potential for performance awards</p> <p>Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Licensure: 1 full and unrestricted license from any US State or territory</p> <p>Work Schedule: Monday - Friday, 0730 - 1600</p> <p>Compressed/Flexible: Not Available</p> <p>Telework: Ad-hoc, according to agency policy</p> <p>Virtual: This is not a virtual position.</p> <p>Relocation/Recruitment Incentives: Not Authorized</p> <p>Permanent Change of Station (PCS): Not Authorized</p>
