<p>Job Description:</p> <p>Newtown Public Schools</p> <p>Transition Coordinator</p> <p>June 20th, 2025</p> <p>POSITION: Transition Coordinator (11 month)</p> <p>LOCATION: Newtown Community Partnership (NCP) Ages 18-22</p> <p>STARTING DATE: August 20, 2025</p> <p>QUALIFICATIONS: Bachelor's Degree. In-depth knowledge of post-secondary resources and transition services.</p> <p>RESPONSIBILITIES: The primary goal of the Transition Coordinator is to assist students with achieving their post-secondary goals through assessment of skills, development of a transition plan, identifying job sites and placing students based on strengths and preferences, and supporting families through the linking of resources.</p> <ul> <li>Coordinates transition activities for college, career, and community readiness </li><li>Develops curriculum, designs and implements lesson plans, and assesses student progress. </li><li>Facilitates referral, application, eligibility process for student and families to adult service providers. </li><li>Acts as a liaison between the school and community agencies such as Bureau of Rehabilitation Services (BRS) and Department of Disability Services (DDS). </li><li>Develops and sustains collaborative relationships with potential job sites. </li><li>Provides direct instruction and consultation to students with disabilities that is designed to meet their individual transition needs. </li><li>Assists with the development of transition goals and attends IEP meetings. </li><li>Oversees and trains job coaches who support students placed in job sites </li><li>Willingness to obtain V-endorsement license (at District expense) </li><li>Performs other related duties as assigned </li><li>Assists in the supervision of transition activities under the direction and supervision of the Director of Pupil Services and Special Education Supervisor. </li></ul> <p>REMUNERATION: $70,000 to $75,000.</p> <p>APPLICATIONS: Please apply through the following link www.applitrack.com/newtown/onlineapp/ by 7/11/25</p> <p>The Newtown Public School District values the benefits that a diverse workforce brings to our students and community.
