<p>Job Title: Director of Social Services Effective Date: 8/01/2012</p> <p>Department: Social Services</p> <p>Supervised By: Executive Director</p> <p>Supervises: Social Services</p> <p>Assistant</p> <p>General Description</p> <p>Under the direction of the Executive Director, the Social Service Director is responsible</p> <p>for monitoring the residents' mental and psycho-social needs and to provide the services</p> <p>to meet these needs in order to attain or maintain the highest practical level of physical,</p> <p>mental, and psycho-social well-being.</p> <p>Essential Duties</p> <ol> <li>Acts appropriately under the direction of the Executive Director and as an active </li></ol> <p>member of the interdisciplinary team.</p> <ol start="2"> <li> <p>Adjusts to changes in schedule to meet resident and family needs.</p> </li><li> <p>Develops a social history of each new resident.</p> </li><li> <p>Utilizes the Resident Assessment Instrument (RAI) process in conducting a</p> </li></ol> <p>psycho-social assessment.</p> <ol start="5"> <li>Formulates a Care Plan which addresses the identified problems, needs, and </li></ol> <p>concerns.</p> <ol start="6"> <li> <p>Documents progress toward goals, assessment updates, and interventions.</p> </li><li> <p>Reviews the resident's progress toward resolution of problems, needs, or</p> </li></ol> <p>concerns; evaluates the effectiveness of the staff approaches; evaluates changes in</p> <p>the mental and psycho-social assessment.</p> <ol start="8"> <li>Participates in the interdisciplinary assessment reviews and revisions; revises the </li></ol> <p>residents' Care Plan according to residents' needs.</p> <ol start="9"> <li>Participates in the pre-admission screening of potential residents and provides </li></ol> <p>information to residents and families.</p> <ol start="10"> <li>Orients the resident to the long-term care environment and facilitates adjustment </li></ol> <p>to placement.</p> <ol start="11"> <li>Coordinates the Complaint / Grievance program with appropriate disciplines and </li></ol> <p>verifies that complaints / grievances are handled in a timely manner.</p> <ol start="12"> <li>Works with residents to obtain Advance Directives upon admission and reviews </li></ol> <p>with residents at least annually.</p> <ol start="13"> <li>Verifies that a Heart-To-Heart coordinator program is implemented in the </li></ol> <p>Facility.</p> <ol start="14"> <li> <p>Reviews behavior charting so that an appropriate Care Plan is developed.</p> </li><li> <p>Provides therapeutic counseling to residents and family members, as appropriate,</p> </li></ol> <p>to meet the residents' psycho-social needs.</p> <ol start="16"> <li>Coordinates appointments in the community for various health care visits and </li></ol> <p>therapeutic needs.</p> <ol start="17"> <li>Coordinates in-services for staff in areas concerning the mental and psycho-social </li></ol> <p>needs of the resident.</p> <p>HISTORY:</p> <p>(A) 8/2012</p> <ol start="18"> <li>Educates the community regarding aging, rights of residents, Facility services, </li></ol> <p>and other related topics.</p> <ol start="19"> <li>Informs residents and families of their rights and responsibilities and educates </li></ol> <p>staff about residents' rights and Facility implementation of those rights.</p> <ol start="20"> <li> <p>Serves as the resident's advocate.</p> </li><li> <p>Assesses the resident's potential for discharge as a member of an interdisciplinary</p> </li></ol> <p>team. Plans for the resident's discharge and post-discharge with the resident,</p> <p>family, and interdisciplinary team. Assists the resident and family in the</p> <p>implementation of the discharge plan.</p> <ol start="22"> <li>Performs various Quality Assurance audits and is an active member of the Quality </li></ol> <p>Improvement team.</p> <p>Standard Requirements</p> <ol> <li> <p>Supports the Mission, Values and Vision of the Facility.</p> </li><li> <p>Is knowledgeable of resident rights and supports an atmosphere which allows for</p> </li></ol> <p>the privacy, dignity and well-being of all residents in a safe, secure environment.</p> <ol start="3"> <li>Supports, cooperates with, and implements specific procedures and programs for: </li></ol> <p>OSHA and safety, including universal precautions and safe work practices,</p> <p>established fire/safety/disaster plans, risk management, and security, report and/or</p> <p>correct unsafe working conditions, equipment repair and maintenance needs.</p> <ol start="4"> <li>Adheres to: </li></ol> <p>a. Confidentiality of all data, including resident, employee and operations</p> <p>data.</p> <p>b. HIPAA regulations</p> <p>c. Quality Assurance requirements and compliance with regulatory</p> <p>requirements.</p> <p>d. Compliance with current law and policy to provide a work environment</p> <p>free from sexual harassment and illegal and discriminatory behavior.</p> <p>e. Code of Conduct and Compliance Programs when performing job</p> <p>functions</p> <ol start="5"> <li>Supports and participates in common teamwork: </li></ol> <p>a. Cooperates and works together with all co-workers; plans and completes</p> <p>job duties with minimal supervisory direction, including appropriate</p> <p>judgment.</p> <p>b. Uses tactful, appropriate communications in sensitive and emotional</p> <p>situations.</p> <p>c. Follows up as appropriate with supervisor, co-workers or residents</p> <p>regarding reported complaints, problems and concerns.</p> <p>d. Promotes positive public relations with residents, family members and</p> <p>guests.</p> <p>e. Completes requirements for in-service training, acceptable attendance,</p> <p>uniform and dress codes including personal hygiene, and other work</p> <p>duties as assigned.</p> <p>f. Participates as assigned in the orientation of new staff and demonstrates a</p> <p>positive, helpful, and enthusiastic attitude.</p> <ol start="6"> <li>Immediately reports incidents of alleged resident abuse or neglect or alleged </li></ol> <p>violations of residents' rights to Supervisor or Executive Director.</p> <ol start="7"> <li>Maintains current practice standards or changes by participating in educational </li></ol> <p>opportunities provided by the Facility and outside resources.</p> <p>Other Duties</p> <p>Perform tasks which are supportive in nature to the essential functions of the job, but</p> <p>which may be altered or redesigned depending upon individual circumstances. Performs</p> <p>other work duties as assigned.</p> <p>Qualifications</p> <p>Education/Training: A degree in Social Services or related field (or relevant</p> <p>professional experience to the position), as required by Federal/State guidelines.</p> <p>A.D.A ((Americans with Disabilities Act)(as amended)) REQUIREMENTS: Must be</p> <p>able to bend, stoop, push, pull, and walk and stand for most of the working day. Requires</p> <p>manual dexterity sufficient to operate standard office machines such as fax, telephone,</p> <p>etc. Requires normal range of hearing and vision. Must be free from communicable</p> <p>diseases that pose a "direct threat" to the health or safety of others and which cannot be</p> <p>eliminated by a reasonable accommodation. Must be able to lift a minimum of 30</p> <p>pounds, and be able to carry and transport supplies. Must be able to pass a post-offer, preemployment</p> <p>physical health examination as a condition of employment. Must be capable</p> <p>of performing the essential functions of the job, with or without reasonable</p> <p>accommodation(s).</p> <p>Skill(s): Must possess proficient reading, writing, grammar, and mathematics skills;</p> <p>proficient interpersonal relations and communication skills; decision-making skills, basic</p> <p>computer skills; and be able to operate various medical and/or office equipment. Must</p> <p>demonstrate knowledge of safety techniques. Must be able to communicate in English,</p> <p>both verbally and in writing, and possess sufficient communication skills to perform the</p> <p>tasks required.</p> <p>Experience: One year experience preferred.</p>
