<p>Join us at One Community Health (OCH) and make a difference in your local community!</p> <p>The Registered Nurse (RN) Care Team Case Manager (CTCM) provides services in support of medical providers and the clinical team. Within scope of licensure and responsibilities, the RN CTCM provides care coordination, case management, and acute interventions following established protocols. This position also manages patients through transitions of care and the care continuum.
