<p>Our Homecare RN Benefits Include:</p> <ul> <li>Up to 6% matching on our 403B retirement plan </li><li>Free Employee Assistance Program </li><li>Onsite Team Member Meal Benefit </li><li>Team Member On-site fitness, consignment and pharmacy </li><li>Bi-weekly pay-direct deposit </li></ul> <p>ESSENTIAL FUNCTIONS/DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Provides coordination of client care, evaluates and ensure continuity and quality of care </li><li>Conducts initial and quarterly assessments for home care clients both in-house and off campus. </li><li>Ensures staff understanding of, and compliance with agency policies and procedures. </li><li>Reviews documentation and performs In-home supervisor visits </li><li>Communicates with physicians, staff, family and clients </li><li>Completes chart audit and Discharge summaries for Home Care clients </li><li>Back up to the Clinic Nurse Manager for assistance and coverage </li><li>Performs direct and individualized nursing care to an assigned group of clients. </li><li>Participates in planning, implementing and evaluating care plans and nursing care </li><li>Performs and/or assists with treatments, therapies and procedures </li><li>Administers and records oral, intramuscular and subcutaneous medications in accordance with home care services policies, procedures and physician orders </li><li>Writes initial nursing assessments upon admission, assesses clients conditions and assists with the development of the individual care plans for clients </li><li>Documents observations, treatments, client response to treatment and nursing care. Completes periodic assessments </li><li>Assists in the development, updating of care plans, and IDCP process </li><li>Gives shift report, documents on 24-hour report, processes physician orders, and assists with orientation of new employees </li><li>Participates in continuing education, staff meetings, and other required in-service training </li><li>Supports and adheres to administrative and nursing policies and procedures </li><li>Knows and is able to initiate fire and disaster plans. Recognizes and responds to client emergencies with appropriate measures </li><li>Utilizes resources, materials and equipment in a safe and efficient manner </li><li>Demonstrates dependability by being punctual; maintains scheduled attendance </li><li>Supports and enforces safety and infection control policies and procedures </li><li>Conforms to facility dress code </li><li>Performs routine housekeeping duties including straightening and maintaining service areas and resident rooms </li><li>Cleans, disinfects and maintains resident care equipment as assigned </li><li>Completes all other duties as assigned by supervisor </li><li>C.A.R.E.
