The Social Worker will provide services to residents and families. The Social Worker will be involved with interdisciplinary and interdepartmental programs as they relate to the residents' quality of life.</p> <p>Responsibilities</p> <ul> <li> <p>Assists families in supporting the resident and functions as an advocate for the resident to assure resident rights.</p> </li><li> <p>Assists residents in meeting and solving their social as well as economic needs and problems.</p> </li><li> <p>Provides personal counseling.</p> </li><li> <p>Develops psychosocial and behavioral care plans.</p> </li><li> <p>Responsible for referrals to appropriate agencies or professionals</p> </li><li> <p>Acts as liaison between resident and community service providers</p> </li><li> <p>Will facilitate discharge planning.</p> </li></ul> <p>What you will need:</p> <ul> <li> <p>Master's degree in Social Work is required.</p> </li><li> <p>NYS Social Work Licensure or Limited LMSW Permit eligible is required.</p> </li><li> <p>Minimum of 1 year of relevant experience</p> </li></ul> <p>Pay Range: $30.00 - $43.08</p> <p>Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
