<p>INFANT AND TODDLER PROGRAM</p> <p>GENERAL STATEMENT OF JOB</p> <p>The Service Coordinator assists the families of children eligible for early intervention access supports and services. The Service Coordinator ensures families receive the rights, procedural safeguards, supports and services that are authorized to be provided under Virginia's Early Intervention program. Reports to Local System Manager/Program Manager</p> <p>ESSENTIAL JOB FUNCTIONS</p> <ul> <li>Contact families to schedule initial intake and screening using the Ages and Stages Questionnaire </li><li>Present completed Ages and Stages Questionnaire information to eligibility team. </li><li>Notify families of eligibility status. </li><li>Schedule assessment with family and notify assessment team. </li><li>Schedule and coordinate Individual Family Service Plan (IFSP) meeting upon completion of assessment. </li><li>Complete IFSP with family and assist them in developing outcomes for their child. </li><li>Obtain physician certification for services from child's primary physician. </li><li>Notify service providers when family selects them for services </li><li>Notify service providers of completed IFSP in the electronic client record. </li><li>Notifies finance of signed IFSP. </li><li>Continuous communications with providers and families to monitor status of case. </li><li>Facilitate transition to other services, i.e. Part B, daycare, preschool, when appropriate </li><li>Collaborate with local schools to set up a transition meeting in an effort to ensure a seamless transition. </li><li>Complete all Part C paperwork required for children eligible for service. </li><li>Assist families in identifying resources and supports they may need. </li><li>Assist families in accessing resources and supports. </li><li>Maintain client record and ensure compliance with Part C regulations </li><li>Participate in community child find activities </li><li>Participate in community awareness programs </li><li>Provide community trainings as requested. </li></ul> <p>MINIMUM TRAINING AND EXPERIENCE</p> <ul> <li>Bachelors degree in Early Childhood Special Education or related field and 1 year full time professional experience in an integrated preschool program, a clinical setting providing special or developmental education, or treatment oriented services to pre-school age children with physical or mental disabilities or emotional, behavioral disorders. </li><li>Bachelors degree in a related field and two years experience serving individuals with disabilities and their families, preferably in early intervention. </li><li>An individual who is the parent of a child with developmental delay or disability and has worked or experience with early intervention for at least one year. </li><li>Must be certified as an Early Intervention Service Coordinator. </li></ul> <p>EMPLOYEE BENEFITS</p> <ul> <li>Medical, dental and vision </li><li>Flexible Spending & Health Savings Accounts </li><li>401(k), including an employer match </li><li>Generous Paid Time Off plan </li><li>Education Assistance </li><li>Life Insurance </li><li>Employee Assistance Program (EAP) </li><li>Short-term disability (STD) </li><li>Long-term disability (LTD) </li><li>As Grafton is a 501(c)(3) non-profit, you may be eligible for federal student loan forgiveness </li></ul> <p>COMPENSATION</p> <ul> <li>$22.84 up to $34.44based upon experience. </li></ul> <p>If you have issues applying or have any questions about this position, please reach out to us at HR@Grafton.org or 540-542-0200 Ext. 6429.</p> <p>Grafton is an equal employment opportunity employer and tobacco-free workplace.</p> <p>#ProfGrafton</p>
