<p>A unique opportunity to work with both Community Health Services and Health Promotion & Outreach Divisions to coordinate and manage a grant funded program of Community Health Workers (CHW). This will include oversight of CHWs working in partner clinic settings and/or community-based settings focusing on underserved individuals and/or families related to Maternal Child Health and Chronic Disease Management. The position has responsibility for planning, organizing, and directing the work of the CHWs to use a case management approach in helping individuals who are pregnant and/or parenting or individuals experiencing one or more chronic disease conditions. Responsibilities include leading a team of CHWs, coaching, problem-solving, and developing and maintaining strong internal and external partnerships to achieve program goals and positive health outcomes. It is beneficial to have experience in supervision and program management as well as experience working with any or all the following: pregnant and/or parenting women and their families, persons with chronic health conditions, and individuals/families with multiple needs. Excellent organization and communication skills and considerable knowledge of local agencies and resources is desirable.</p> <p>Applicants with interpretation skills and from diverse backgrounds are encouraged to apply.</p> <p>Desirable Knowledge, Abilities & Skills/Desirable Training & Experience:</p> <p>Ability to lift and carry items weighing up to 50 lbs.</p> <p>Possess Valid driver's license</p> <p>Minimum Qualifications</p> <p>Graduation from an accredited four-year college or university with major coursework in business or public administration, public health administration, the social or behavioral sciences, or related field, plus four years of experience in a supervisory capacity related to the position and program to which assigned; or any equivalent combination of training and experience that provides the desirable knowledge, abilities and skills.</p> <p>1.0 full time equivalent with benefits. Evening and weekend hours are possible; majority of hours from 8 a.m. to 4:00 p.m., Monday - Friday.</p>
