<p>Job Title: RN Care Manager</p> <p>Job Description</p> <p>The RN Care Manager provides comprehensive clinical assessment, member-centered care planning, and ongoing care coordination for program members, including older adults and individuals with intellectual or physical disabilities. This role focuses on achieving meaningful member outcomes through evidence-based standards of care, close collaboration with members and their families, and continuous quality improvement in all aspects of service delivery.</p> <p>Responsibilities</p> <ul> <li>Perform discipline-specific initial and ongoing comprehensive clinical assessments for program members. </li><li>Develop, implement, and maintain individualized, member-centered care plans that promote optimal health, safety, and quality of life. </li><li>Conduct home visits and home assessments as needed, including visits to members in long-term care (LTC) facilities. </li><li>Consult with families, caregivers, and other support persons to gather additional information and ensure coordinated care. </li><li>Use critical thinking and clinical judgment to optimize members' health outcomes and help them achieve their personal goals. </li><li>Educate members and their families about health conditions, treatment options, care plans, and available community resources. </li><li>Coordinate services across the continuum of care, including medical, behavioral health, and social support services. </li><li>Monitor the quality, effectiveness, and appropriateness of services provided to members, and adjust care plans as needed. </li><li>Identify and participate in initiatives to continuously improve the quality of member care and services. </li><li>Interact with members and their families with a high level of respect, compassion, and integrity, reinforcing a strong member-centered culture. </li><li>Support operational goals by adhering to operational standards and complying with contractual, regulatory, and internal policy requirements. </li><li>Participate actively in training, orientation, and ongoing professional development opportunities. </li><li>Engage in efforts to enhance team collaboration and overall work experience. </li><li>Ensure accurate, timely documentation in healthcare management and care management systems, including prior authorizations when applicable. </li></ul> <p>Essential Skills</p> <ul> <li>Active LPN or RN license in the state of Wisconsin. </li><li>LPN or RN degree from an accredited nursing program. </li><li>Certification as a Long-Term Care Functional Screener / Certified Functional Screener. </li><li>Minimum of 2 years of care management experience. </li><li>Demonstrated experience in case management and care coordination. </li><li>Knowledge of Medicare, Medicaid, and managed care programs, including BadgerCare. </li><li>Ability to develop, implement, and manage individualized care plans. </li><li>Strong clinical assessment and critical thinking skills. </li><li>Ability to work effectively with older adults and individuals with intellectual or physical disabilities. </li><li>Proficiency using healthcare management and care management systems. </li><li>Strong communication and education skills for working with members and families. </li><li>Ability to comply with contractual, regulatory, and organizational policies and procedures. </li></ul> <p>Additional Skills & Qualifications</p> <ul> <li>Experience in social work, psychology, or human services. </li><li>Experience in the healthcare industry or healthcare management. </li><li>Familiarity with prior authorization processes. </li><li>Commitment to a member-centered approach that emphasizes respect, compassion, and integrity. </li><li>Interest in continuous learning and participation in training and professional development. </li><li>Ability to work both independently and collaboratively within an interdisciplinary team. </li><li>Strong organizational skills and ability to manage a flexible schedule based on member needs. </li><li>Motivation to grow within an organization that supports internal advancement based on performance and organizational needs. </li></ul> <p>Work Environment</p> <p>This position serves members in Sheboygan County. The RN Care Manager manages their schedule based on member needs, with primarily day-shift hours. The role requires regular in-home visits and visits to long-term care facilities. Team members may work from home and also have access to an office for training, internal meetings, and ongoing collaboration. Initial training and orientation occur primarily in the office, generally for the first 3-6 months, after which, as proficiency and trust are established, the RN Care Manager may work remotely on some days. On an ongoing basis, staff are typically in the office about three days per week. Training and orientation will take place in Milwaukee at the Vliet location, with required attendance for orientation and up to four days on-site; mileage for this travel is reimbursed. The environment combines field-based work, office-based collaboration, and remote work flexibility, supporting professional development and internal advancement opportunities.</p> <p>Job Type & Location</p> <p>This is a Contract to Hire position based out of Sheboygan, WI.</p> <p>Pay and Benefits</p> <p>The pay range for this position is $32.69 - $38.46/hr.</p> <p>Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
