Join Easterseals NH as a Caregiver Specialist and help empower individuals and families to live fulfilling, independent lives.</p> <p>Easterseals NH is seeking an energetic, knowledgeable, and flexible individual to join our team at the Aging and Disability Resource Center (ADRC), formerly known as ServiceLink. The ADRC helps individuals understand and access community-based long-term services and supports. Services include caregiver support through the New Hampshire Family Caregiver Program, assistance with state program applications, and guidance for individuals navigating Medicare.</p> <p>This position will work under the New Hampshire Family Caregiver Program and will be responsible for developing and facilitating caregiver support groups in the Manchester and Nashua areas. The Caregiver Specialist will be trained and certified in evidence-based caregiver education programs such as Powerful Tools for Caregivers and SAVVY. Once certified, the individual will establish support groups and facilitate caregiver training sessions throughout the year.</p> <p>Additional responsibilities include supporting the Caregiver Specialist team through bimonthly caregiver check-ins, answering incoming calls, and assisting with intake, referrals, and general office support.</p> <p>Hours: This is a full-time, 40 hour/week position; 8:00am to 4:30 or 8:30 to 5pm.</p> <p>Travel: Nashua and Manchester offices</p> <p>Compensation: $21.00 per hour</p> <p>Responsibilities:</p> <ul> <li> <p>Provides ongoing support and services to caregivers under the mandates of the NH Family Caregiver Support Program</p> </li><li> <p>Provides ongoing support to caregivers through support groups and trainings.</p> </li><li> <p>Provides information on and referral to community resources which may meet the needs of the caregiver</p> </li><li> <p>Acquires through conversations with the caller, an assessment of the contact/caller's situation/needs and provides the contact/caller with assistance in referrals to long-term supports and services through telephone contacts, walks-ins, emails, home visits/community settings and multi-media.</p> </li><li> <p>Provides information and education on community long term support and services to individuals in hospitals, rehab facilities, and/or nursing homes as mandated by the contract.</p> </li><li> <p>Completes eligibility screening and provides information to assist individuals in understanding and applying for State and Federal program.</p> </li><li> <p>Completes documentation on all contacts using assigned database following all protocols/processes as mandated by the contract and in compliance with InformUSA.</p> </li><li> <p>Complete follow-ups on contacts following InformUSA protocols and contract requirements.</p> </li><li> <p>Comply with completion of reporting requirements mandated by the ADRC contract.</p> </li><li> <p>Provide Medicare information and counseling to Medicare beneficiaries especially during the Medicare Annual Enrollment Period.</p> </li><li> <p>Attends meetings as mandated by contract or required by State.</p> </li><li> <p>Assists with general office support including by not limited to answering phones, assisting, walk-ins and other office tasks as needed.</p> </li><li> <p>Obtain all mandatory certification as required by the contract which includes the InformUSA (Alliance for Information and Referral Certification (AIRS)); State Health Insurance Assistance Program (SHIP) Counselor within six (6) months to one (1) year of hire.</p> </li><li> <p>Perform additional duties as requested.</p> </li></ul> <p>Qualifications:</p> <ul> <li> <p>Bachelor's Degree in Social Work, Education, Human Services, Sociology, Cultural Anthropology, or Behavioral Science, OR Eight (8) years' experience working directly with programs supporting the elderly or disabled population in lieu of degree</p> </li><li> <p>Basic knowledge of community long-term care support services for the elderly and disabled population OR combination of education and experience totaling eight (8) years,</p> </li><li> <p>Basic knowledge of community services such as home health agencies, assisted livings, nursing homes, and hospitals.</p> </li><li> <p>Basic knowledge of State and Federal program such as Medicaid and Medicare.</p> </li><li> <p>Strong oral, written and interpersonal skills.</p> </li><li> <p>Valid driver's license, auto insurance, and reliable transportation.</p> </li><li> <p>Analytical and problem-solving ability.</p> </li><li> <p>Comfortable with performing home visits and community outreaches.</p> </li><li> <p>Basic level of computer skills to include Microsoft Office application suite and database programs</p> </li></ul> <p>What's in it for you?
