<ul> <li> <p>JOB REQUIREMENTS</p> </li><li> <p>Education</p> </li><li> <p>Graduation from an accredited school of pharmacy with Doctor of Pharmacy (PharmD) degree required. Some exposure to chemical hazards when making chemotherapy.</p> </li><li> <p>JOB SUMMARY</p> </li><li> <p>The Transitions of Care Pharmacist is responsible for clinical aspects of pharmaceutical patient care including interaction with patients, families, physicians, nurses, and other healthcare providers. As a key member of the treatment team, the Pharmacist will optimize medication therapy management to improve clinical and economic outcomes and ensure a patient's pharmaceutical plan of care is fully implemented through transitions of care. Activities will encompass participation in admission medication reconciliation, discharge medication reconciliation, patient counseling/teaching and multidisciplinary care planning.</p> </li></ul>
